The influence of physiotherapy for patients with ankylosing spondylitis treated in outpatient facilities and spa resorts

This Bachelor´s thesis is concerned with influence of physiotherapy at patients with Bechterew´s disease, out of them one group of research sample undergoes therapy in outpatient facilities and the second one undergoes all-embracing balneotherapy. In theoretical part is briefly described the stability of backbone, functional anatomy and kinezilogy of axial system. Next part is concerned to a detailed description of Bechterew´s disease, concerned with its categorization to the group of serum-negative spondylartritid, with definition and classification of the disease, its occurrence, genesis and progress, prognosis and clinical picture of the disease. The main aim of this thesis is to evaluate the effects of physiotherapy on patients with Bechterew´s disease who were treated in outpatient facilities and spa sanatorium. To achieve the goal was used the qualitative research, in which four participants took part, out of them two were treated outpatiently, namely in Rehabilitation and regenerative Borovany and in Outpatient clinic Medipont, s.r.o. Next two participants attended complex curative therapy in Berta´s spa Třeboň. The patients had different stadium of the disease in the range between II. V. stage of disability. During the introductory meeting was executed preliminary kinesiological examination, next they were asked to fill up the informed agreement and Functional index BASFI (The Bath Ankylosing Spondylitis Functional Index), which is used to considering the patient´s functional abilities to solve common daily activities and which reflects impact of the disease to the quality of patient´s life. After therapy´s ending it was executed final kinesiological analysis together with repeated filling out the Index, in order to evaluate success rate of the treatment in range of theirs functional skills. With participants were as well executed not structured interview, in which they communicate their subjective evaluation to the realized research and compared outpatient and spa therapy from their pont of view to their contribution to themselves. The results of preliminary and final researches and physiotherapy course are placed upon record in practical part of Bachelor´s thesis. The Bachelor´s thesis can be used as study material for students, medics and non-professional public as well to improve knowledge about influence of the physiotherapy at patients with Bechterew´s disease, as well to teach patients about possibilities of their treatment.
Possibilities and difficulties of lifelong education for nurses in outpatient departments

The thesis is focused on educational possibilities for nurses working in outpatient departments. Professional development of nurses is made possible by lifelong learning. It is a way how to obtain current information, keep up with recent knowledge and skills that may be applied in medical care. The results have shown that both nurses in private outpatient departments and nurses in policlinics choose the seminars or lectures according to the topic, not according to the place where they are held. The majority of nurses from private outpatient departments and policlinics choose seminars and conferences relevant to their professional and personal interest, not according to the price. Bosses in policlinics are more obliging when nurses wish to attend seminars and conferences than private doctors.
Sampling of biological material in outpatient health care facilities viewed by a nurse.

Sampling of biological material is one of the most frequent duties of nurses working in outpatient health care facilities. The nurse takes biological samples daily and has to respond to possible problems. Sampling of biological material is an important component of the patient´s examination and therefore it must be carried out correctly and properly. The quality of the procedure is affected by the nurse´s experience, skills and knowledge. It is important for a nurse working in an outpatient health care facility to master the sampling technique so that patients were satisfied and the samples did not get devaluated. The objective of my research was to give an account of the most frequent problems occuring when samples of biological material are taken in outpatient health care facilities, and to find out if nurses give patients enough information before the procedure. I set three hypotheses: the H I - nurse do not use protective aids while taking samples of biological material, the H II {--} nurse do not follow the recommended standards, the H III {--} nurse do not inform patients about preparations before taking samples of biological material. To achieve the objective and to verify the hypotheses the questionnaire method was used. Questionnaires were addressed both to patients and nurses. The research sample included 103 outpatients and 73 nurses from outpatient health care facilities in the South Bohemian and the Central Bohemian regions. The obtained data analysis showed that the H I {--} nurse do not use protective aids while taking samples of biological material {--} was not confirmed. H II {--} nurse do no follow the recommended standards {--} was not confirmed. The H III {--} nurse do not inform patients about preparations before taking samples of biological material {--} was not confirmed either. The results of my research reflect the fact that the most common problem is communication between a nurse and the patient. But there is more to be improved - patients education, the fixed placement of aids and the early transportation of samples to laboratories. I would like to make use of these conclusions and pass them over not only to nurses who took part in the research but also to the hospital managements. I intend to give a presentation on my research conclusions to nurses working in outpatient health care facilities.
