Specifics of nursing in internal medicine office

This thesis deals with the specifics of the work of nurses in the internal clinic. The internal clinic is one of the most important part in the health sector. Human population is aging and with age a lot of diseases appear. An illness causes many obstacles. A bad health condition also negatively affects family and social life. The illness may be the limit in job duties or in recreational activities. For patients it is very important to get an adequate care from qualified medical personnel. This work is divided into two parts. The first part focuses on the theoretical description of problems, the definition of internal medicine and the history of nursing in internal medicine. It also outlines the most common diseases in the internal clinic, an internal organization and the types of outpatient departments. Another important part of the theory part is to characterize the personality of nurses and activities which they do in the clinic. The second part of the thesis is the empirical part, which examines the work of nurses in the clinic. For the practical part it is outlined a goal aimed at mapping out the specifics of work of nurses in internal clinic. To achieve the goal it was used a qualitative research. The research consisted of interviews with nurses from internal ambulance in the Hospital Czech Budweis and the Hospital Prachatice. The interviews were divided into three categories. The first one categorizes respondents, the second category deals with work and communication of nurses with patients. The last category monitored the presentation of nurses in internal clinic. The research group consisted of 12 respondents. Respondents were classified as R 1-12. Interviews were recorded, transcribed verbatim and then analysed. Data collection took place in March 2015. For the research survey it was chosen a research question which dealt with the specifics of work of nurses in internal clinic. Based on the survey it was found out that the most common outputs in internal clinic are withdrawal the biological material, measuring physiological functions and application of injections. The specific outputs of nurses include gastrolaxage and bandaging of wounds and lesions. The clinic regards good communication between the patient and the nurse and the nurse and doctor. The research also showed that respondents from both chosen hospitals operate according to the standards and never exceeded their authorities. At the same time, however, they indicated that the work is time-consuming, because it is difficult for them to take care of the patient effectively enough. Nurses need some time on cooperation with the physician. Other frequent outputs of nurses include the work with documentation. Based on the analysis of data it was created a recommendation for nurses working in internal clinic, which should contribute to improved efficiency of nursing care and help nurses with the problem of absence of time.
Medical Emergency from a Nurse´s Point of View.

The nurse?s approach to a patient who is being admitted at the emergency ward should be in all circumstances in compliance with the holistic philosophy. In the thesis we focused on the emergency admission of a patient from the nurse?s point of view. The emergency admission of a patient requires a quick and professional approach. It is important for each nurse to realize that communication with the patient, the doctor and other members of the medical team is the part of this professional approach. Another essential part is an active search of patient?s current problems. In order to manage this, it is necessary to show the ability to cooperate with the patient, to know, how to proceed in a physical checkup, to be able to describe the deviations from the standard, and last but not least, to be able to communicate with the doctor about the findings. The theoretical part of the thesis involves chapters dealing with holism, holistic philosophy, communication and physical check-up of a patient. Further this part deals with getting the anamnesis, the agreement with the admission to hospital and other formalities that belong to the procedure of admission of a patient. At the end of the theoretical part we characterize the surgical ward and the internal ward and describe abdomen aches and chest pains as typical examples of emergency admission reasons at these two wards. The objective of this work is to find out whether nurses at these emergency wards (surgical and internal) during the process of admission provide a patient with care that is in compliance with principles of holistic philosophy and also to compare the holistic approach of nurses at the surgical and internal emergency wards. 4 questions were set before the research started. Research question No.1: How are biopsychosocial needs of a patient at the emergency admission saturated? Research question No.2: How can the communication of a nurse with a patient at the emergency admission ward, during the admission procedures, influence the following cooperation of the patient with the medical staff? Research question No.3: What does the difference between the holistic care at the surgical and internal ward lie in? Research question No.4: How can the circumstances during the emergency admission of a patient influence a nurse?s respect of patient?s needs? Half-standardized interviews with 3 nurses from the surgical ward and with 3 nurses from the internal ward were used in the research part. Then, a concerned observation of those nurses was carried out, while they were taking care of a patient at the emergency admission ward. On the grounds of the research results we can assume that the objective of the thesis was met. The objective was to describe different views and approaches of nurses to a patient who is being admitted at the emergency ward. The research results were used to compose an instruction manual called: What nurses at the Emergency Admission Ward should not forget.
