Suturing an obstetric injury in the competences of a midwife

The bachelor thesis deals with the topic of perineal laceration repair within the scope of a midwife's competence. The aim of the thesis is to create an educational video that can serve as instructional material for midwifery students. Fulfilling the main objective of the thesis is related to achieving the following specific goals: creating a comprehensive theoretical compilation of available resources related to the topic of the thesis, developing a video script, and capturing footage with expert personnel. The thesis thoroughly examines the anatomy of soft tissues and subsequently focuses on the profession of midwifery, including competencies and education. In further sections, it explores the diagnosis and prevention of childbirth injuries, encompassing practices for both women before childbirth and midwives during labor. Simultaneously, it describes the procedures for treating injuries and the relevant materials and tools. The conclusion of the thesis addresses the subsequent care of injuries during the postpartum period and possible complications. The accompanying educational video addresses topics such as postpartum genital examination and injury management, depicting and describing them in detail with voiceover commentary. The impact of episiotomy on women's health

My bachelor thesis deals with the issue of episiotomy and its impact on the health of women who have given birth spontaneously. An episiotomy is an obstetric procedure that was often routinely performed in the past, but in recent years has become the subject of research regarding its long-term consequences on women's health. My thesis consists of two parts - theoretical and research. The theoretical part includes anatomy of the pelvic floor, definition of episiotomy and its historical development. It also contains information about the possibilities of prevention of birth injury and faster recovery from the resulting injury. The research part is developed in the form of an online questionnaire that was posted on social media groups related to pregnancy, childbirth and postpartum. The questions were formulated in such a way that women could choose from multiple options or provide open-ended answers. The questionnaire was intended only for women who had given birth spontaneously. The main aim of the research was to find out how episiotomy affects the physical, psychological and sexual aspects of a woman's life compared to women who did not have an episiotomy. Birth injuries and their impact on women

The bachelor thesis is focused on the topic of birth injuries and their impact on women. The thesis includes a theoretical and practical part. The theoretical part of the bachelor thesis describes the types of birth injuries, the reasons for their occurrence and preventive methods. Attention is also given to the diagnosis and treatment of birth injuries and the care of a midwife for birth injuries during the six-week postpartum period. At the end of the theoretical part are summarized the most common impacts on women related to birth injuries. For the practical part of the bachelor thesis were established three objectives and four hypotheses. The first objective was to determine the most common problems women face in relation to third and fourth degree perineal tears. For this objective were determined two hypotheses. H1: Women with third and fourth degreee perineal tears have problems in their sex life. H2: More than 75% of women with third and fourth degree perineal tears experience unwanted gas leakage. The second objective of the bachelor thesis was to map out the role of a midwife in caring for birth injuries. One hypothesis was established for this objective: H3: The role of a midwife in caring for birth injuries involves educating women in this area. The purpose of the third objective was to determine women's awareness in caring for their birth injuries. For this objective was chosen one hypothesis: H4: Women obtain information regarding the care of birth injuries more from midwives than from doctor. The goals were achieved through quantitative research using online questionnaires. The questionnaire contained 18 questions: six closed questions, eleven semi-closed questions where multiple answers could be selected, including an "other" option, and one open-ended question. Statistical processing of all questionnaires was done in Microsoft Excel. The research sample consisted of 620 women who gave birth between March 2019 and January 2024, and who had third and fourth degree perineal tears or an episiotomy during childbirth. Out of the total of 620 respondents, birth injuries had an impact on 446 respondents, while 174 respondents did not experience any problems related to birth injuries. The research revealed that birth injuries (episiotomy and third and fourth degree perineal tears) had an impact on more than 70% of respondents. It was also found that almost 35% of respondents were not educated on the care of birth injuries during hospitalization. Out of the four hypotheses, hypotheses H1, H3 and H4 were confirmed, while hypothesis H2 was not confirmed.
Epi-no (Aniball) balon as a preventiv of birth injuries.

This bachelor's thesis is focused on a prenatal preparation, specifically on how to prevent mother's injuries during childbirth after she used Epi-no (Aniball) during pregnancy. The thesis is divided into two parts: theoretical and practical. The theoretical part is written in four chapters. The first chapter deals with all birth stages. The second chapter describes mother's birth injuries. The third chapter is focused on the episiotomy itself, and the fourth chapter discusses the prevention of mother´s birth injuries. The first goal of the thesis is to find out women's experiences with the use of Epi-no (Aniball) vaginal dilatation balloons. The second aim of the thesis is to find out the midwives' opinions on exercises with the Epi-no balloon (Aniball). Qualitative research is conducted in the form of semi-structured interviews with respondents. Two research groups are identified. The first research group consists of postpartum women from the South Bohemian Region who were using vaginal dilatation balloons (VDB) during pregnancy. In the second research group are midwives who have practical experience with VDB. Due to the verbal nature of the research, the collected data were transcribed into printed form. Then the data were processed and analysed using the pencil and paper technique. In order to maintain anonymity, the mothers were randomly labelled as informants R1 to R10. Midwives were randomly identified as informants PA1 to PA6. As part of the coding of interviews with women, 1 main category and 5 subcategories were defined. By coding interviews with midwives, 1 main category and 2 subcategories were defined. Based on the stated goals of the work, two research questions are determined for the practical part. The first research question investigates what experience women have with the use of the Epi-no balloon (Aniball) in the prevention of birth injuries. The research is leadingto the conclusion that some mothers didn't know whether they were performing the exercises with the VDB correctly. On the other hand, the exercise with the balloon helped them to figure out what position is most comfortable for them when pushing the baby out and how to engage the abdominals when pushing. It also gave them self-confidence during childbirth. The second research question asked for midwives' opinions about the use of the Epi-no (Aniball) balloon. The research reveals that all midwives recommend using VBD as it is beneficial for mothers before giving birth. Most midwives mentioned the psychological effect of using VDB. By believing that pregnant women are prepared for the birth, they are more calm and relaxed in the delivery room. Midwives also reported the effect of exercise on shortening the II. delivery stage, and in some cases, they observed reduction of number and extent of birth injuries as well. The knowledge gained during the preparation of the bachelor's thesis can effectively help in prenatal preparation courses and can increase awareness of pregnant women about possible preparation for childbirth.
Injury to women in childbirth

My bachelor thesis is focused on various kinds of wounds, diagnostics, treatment and prevention of birth wounds. The theoretical part addresses in particular episiotomy since it is the most frequent wound that has been currently discussed by general public. I describe kinds, indication, treatment, nursing care and prevention of episiotomy. In the practical part I selected a quantitative research. The data were collected by means of an anonymous questionnaire designed for women after spontaneous delivery in puerperal wards in selected hospitals. I set three objectives in my thesis. The first objective was to ascertain the episiotomy rate. The second objective was to ascertain episiotomy healing and the last objective was to reveal awareness of women in respect of episiotomy care. The objectives of the thesis have been fulfilled. The first hypothesis presumed episiotomy in more than a half of the informants. This hypothesis has been confirmed. The second hypothesis presumed episiotomy healing without complications. This hypothesis was also confirmed, showing evidence of a high standard of healthcare. The last hypothesis presumed that the informants had insufficient information about episiotomy care. As the result of a positive response provided by mothers themselves, most of whom stated to be sufficiently informed about episiotomy care, this hypothesis was disproved. My thesis could serve as a suitable study material for students of midwifery. My thesis could also be beneficial for midwives working in puerperal wards and delivery rooms.
Health and social impacts on women after complicated childbirth.

In the diploma thesis, objectives were set that were intended to find out medical, mental and social problems of women after complicated childbirth and to compare the problems with those of women after physiological childbirth. The other objective was to find out what kinds of complicated deliveries occur in Nemocnice Písek, a.s. /Písek Hospital/ the most. Hypotheses were set for the objectives. The first hypothesis was to confirm that birth injuries after complicated childbirth are healed per secundam. The second hypothesis was to confirm that women after complicated childbirth feel that they will not manage care for their family. The third hypothesis was to confirm problems with economic support of a family in the cases of single mothers after complicated childbirth. The fourth hypothesis was to confirm that women after complicated childbirth do not have enough information about social security. The fifth hypothesis was to confirm that in Písek Hospital, the most frequent complicated deliveries include Cesarean sections.
