LINHARTOVÁ, Petra
Abstract One aspect of nursery care, which is still neglected in czech hospitals, is right of pacients to privacy. That means, right to intimacy in various situations as well. Intimacy is sensitive area, that is confirmed especially by pacients. Pacients percieve questions about their sexulatity and emptying very annoyingly, which are included in nursery process. They don´t want answer them and consider them as inadequate. Accordingly it is very important to keep feeling of privacy, even in case of pacient, where is practised just essential nursery intervention, for example purgative clysma. Preamble theoretical part of this thesis discuss about anatomy an fyziology of thick intestin and about history of applying the purgative clysma. There are presented correct methods in applying the purgative clysma, for example right position, place where is clysma applied, indication and contra-indication during the provision purgative clysma. In the second theoretical part of this thesis is explained expression intimacy, areas and factors, which influence the intimacy. There is mentioned role and dilemma of naked, feeling of shame, self-respect and self-confidence. For obtaining of necessary dates to access set targets and to make anaylsis of these dates, was used questionnaire method. Questionnaires were distributed to pacients and nurses at surgical wards in Ceske Budejovice and Chrudim. Main target of this thesis was to determine the satisfaction of clients during supply of intimacy in applying the provision purgative clysma. The first one of defined hypothesis was not confirmed. Clients at hopitals are dissatisfied with supplying of intimacy by nurses during the provision purgative clysma. Hypothesis number two was not confirmed as well. For self sufficient clients is intimacy during the provision purgative clysma better supplied then for bed-rid clients. Results of this thesis demonstrate fact, that self sufficient clients and bed-rid clients are satisfied with methods, which are kept by nurses during the provision purgative clysma.
Nursing care women during wirt abortion
KOUTNÍKOVÁ, Radka
A birth assistant should manage several roles when taking care of women {--} she should communicate well, be a good nurse, should show empathy to women, introduce new trends and knowledge into her work, discuss them with her colleagues, be able to defend her actions and contribute to the improvement of nursing care. My work uses a quantitative survey based on the collection of data through questionnaires combined with a qualitative survey using interviews to clarify survey questions. The survey was carried out from October to December 2008. Research subjects included birth assistants and women patients at Gynaecological and Obstetric Departments. Questionnaires for birth assistants were distributed in 5 hospitals in the Czech Republic (Ústí nad Labem, Jindřichův Hradec, České Budějovice, Třebíč and Brno). I distributed 150 questionnaires for birth assistants; 105 (70%) questionnaires were collected. 15 questionnaires were discarded for being incorrectly completed; the research uses the total of 90 (100%) questionnaires. Interviews with women patients were carried out in the hospital in Ústí nad Labem. I interviewed 8 patients, who had been informed about the structure of the interview. They agreed that the interviews were recorded by dictaphone; the interviews were subsequently transcribed and assessed in charts and tables. My bachelor thesis formulates four targets and four hypotheses. The survey confirmed the targets and the hypotheses. The bachelor thesis deals with women after abortion; it focuses on nursing care, information provided to women patients, the securing of their intimacy and the care rendered to them according to standards. During the collection of data, I did not encounter unwillingness or displeasure on the part of birth assistants or women patients. The approach of both these groups was positive; 70% of birth assistants and 100% of women patients cooperated on the research. Both groups were interested in the results of the research. The results and theoretical part of the bachelor thesis will be used as a source of information for birth assistants and women after abortion. Birth assistants promised to use the results of the work as inspiration for the improving of their care of women after abortion. The bachelor thesis may also be used as an information brochure for women after abortion and the general public. It may provide information for the production of leaflets available at Gynaecological and Obstetric Departments or lectures.
The need for intimacy and security in the provision of nursing care for selected minority roups
PINCOVÁ, Dana
The Need for Intimacy and its Assurance in Nursing Care for Selected Minority Groups The need for intimacy is an important part of everyday life, and it is necessary to assure it at hospitalization. This need is individual, specific to each person and the culture the person comes from. The theoretical part of this thesis is devoted to this issue. The concept of intimacy and the need to assure intimacy in nursing care with a focus on specifics of the nursing care for members of minority groups are described. The master´s thesis objective was to identify the specifics to assure the need for intimacy of selected minority groups and to determine whether nurses know the specifics of intimacy in nursing care for patients from selected minority groups. To meet the objectives, the following hypotheses were stated: H1: There are specifics in the need for intimacy in selected minority groups, and H2: Nurses know the specifics in the need for intimacy in selected minority groups. One research question was asked: Is the need for intimacy in patients from selected minority groups assured at hospitalization? To conduct the diploma thesis research, a quantitative-qualitative survey was selected and the interview method was used ? questionnaires for nurses and the general public, and the interview technique for representatives of selected minority groups. The research set consisted of nurses in hospitals, the general public and representatives of selected minority groups. The survey was carried out during the months of February and March 2011. Specifics in the need for intimacy assurance in selected minority groups exist, and nurses are familiar with them. They know them mainly form their nursing practice when they have met a member of a minority group. The answer to the research question was unequivocal. Yes, the need for intimacy in patients from selected minority groups is assured. It is assured primarily by using the fittings of individual departments, which are the curtains or blinds, as well as by a helpful and sympathetic attitude of the medical staff who try not to unnecessarily unveil the patients´ bodies and according to available means to provide them with as much privacy as possible either during examination, a treatment intervention or while on bed rest. The outcomes show that both objectives were met and the hypotheses were confirmed. The research results were the basis for elaboration of an informational brochure for nurses, which may help in nursing care for minority groups members. And not only for assuring the needs for intimacy, but for a general overview of specifics in nursing care for minority groups members.
Obstipacion as a problem in nursing
PETRŮ, Štěpánka
Obstipation as a problem in nursing. A growing portion of today{\crq}s population suffers from obstipation. Longer life expectancy and today{\crq}s lifestyle are seen as the most frequent reasons for it. Obstipation is a complaint nurses encounter in all kinds of health care institutions. Providing suitable nursing care may help the patient with defecation. The first part of the thesis is theoretical. It provides the definition of the disorder, its causes and proper nursing method, which is listed in chronological order according to the nursing procedure. The next part deals with a survey, which was carried out in hospitals in Pelhřimov, Jindřichův Hradec and Počátky at departments where higher incidence of obstipation is anticipated. The survey was anonymous, and the data were collected from 128 nurses in a quantitative research using the method of questionnaire. In order to remain objective, structured interviews containing the same questions were made with 11 inpatients hospitalized at the departments included in the survey. The thesis followed three goals. First, to find out whether nurses make an inquiry about obstipation that is to be included in the patient{\crq}s history. Second, to map out nursing care in patients with obstipation at departments with beds. Third, to identify current options nurses can choose from while dealing with obstipation as a problem in nursing. Based on the goals, three hypotheses were determined. First, nurses are concerned about the frequency of patient{\crq}s defecation at their homes while collecting details for patient{\crq}s history. Second, nurses do not keep the patients hydrated enough. Third, nurses secure intimate place for defecation when requested by the patient. Nurses themselves confirmed the first and third hypotheses and refused the second one. The patients` answers did not always match with nurses` responds. In conclusion, both surveys proved that nurses have theoretical knowledge related to looking after a person suffering from obstipation but they are not always ready to implement such knowledge. The thesis provides an overview of the current nursing care in patients with obstipation and is set to help implement a better and more effective usage of nursing methods in practice.
Observance of elderly patients rights in long-term facilities.
HAVRDLÍKOVÁ, Markéta
The topic of this diploma paper is ``The Observance of the Rights of Senior Citizens at Long-Term Care Wards{\crqq}. The growing number of senior citizens brings the increasing number of hospitalized people at wards of long-term (subsequent) care. The paper focuses on the observance of seniors{\crq} rights by the attending staff at these wards. It is divided into two sections {--} a theoretical one and a practical one. The theoretical section describes patients{\crq} rights in relation to ethics. It develops the Ethics Codex of Patients{\crq} Rights, the European Charter of Senior Patients. It also discusses patients{\crq} rights in relation to the legislature. It deals with individual laws and their observance by the attending staff that is involved in the care of senior citizens at long-term care wards. It describes the duties of the staff based on individual laws. The practical section sets the objectives focusing on finding out about the observance of senior patients{\crq} rights at long-term care wards as seen by general nurses, senior citizens, and family members. It also attempts to find out about the most frequent forms of non-observance of seniors{\crq} rights. To achieve these objectives, a quantitative research survey was used. The techniques of data collection from the senior citizen respondent group included a structured interview, and a questionnaire for the groups consisting of nurses and family members. The survey resulted in the finding that, according to the respondents, (senior) patients{\crq} rights are always, or almost always, observed in most domains. In spite of this, the respondents perceive some problems in the domain of respecting patients{\crq} shyness and attending staff{\crq}s anonymity. Only some members of staff introduce themselves to patients upon their first meeting, and seniors know the names of only a few attending team members. In the course of hospitalization, a senior patient is not only a recipient of care but also a partner to the attending staff. To provide quality nursing care, which is highly emphasized these days, the knowledge of (senior) patients{\crq} rights by attending staff is essential. Only a senior citizen who knows their rights as well as an attending person is able to protect fully the hospitalized seniors{\crq} rights. The results of this diploma paper can be used in seminars, conferences for medical staff to deepen the knowledge of (senior) patients{\crq} rights. Similarly, senior citizens themselves will be made more aware of the rights they have in relation to receiving medical care.
Providing the patient´s privacy after total hip replacement operation
RŮŽIČKOVÁ, Michaela
Abstract In our company the emphasis in the field of nursing care has increasingly been focused on preservation of a patient`s dignity and intimacy. In daily activities in a pacient the privacy and intimacy the patient may be affected which causes different reactions of such a patient. One might say that even after the routine procedure such as total hip replacement it is very important that the patient has enough privacy. The aims of this study were to assess how the intimacy in the postoperative care is ensured in patients undergoing total hip replacement at the intensive care units and to find out the level of knowledge of the nurses as for adherence to intimacy after total hip replacement. The research group consisted of patients who were hospitalized at the intensive care and standard units in the Hospital České Budějovice, a.s. (PLC), Faculty Hospital Olomouc and Military Hospital Olomouc. The second research group consisted of the nurses who worked at the intensive care and standard units in the Hospital České Budějovice, a.s. (PLC), the Hospital Šumperk, a.s., Faculty Hospital Olomouc and Military Hospital Olomouc. The research investigation was performed by the method using two ananymous questionnaires. The first defined hypothesis was confirmed. The patients at the intensive care units are more satisfield as for their intimacy compared to the patients at the standard units. The second hypothesis was also confirmed. The nurses have not enough time to ensure intimacy of the patient after total hip replacement. The third hypothesis was confirmed. The nurses have enough information of how to take care of the intimacy in the postoperative period. The fourth hypothesis was not confirmed. The nurses at the intensive care unit do not care about the intimacy with a higher interest compared to the nurses at the standard unit. The findings of this investigation show that the patients after total hip replacement at the intensive care unit are more satisfield with the provision of intimacy in the postoperative care compared to patients at the standard unit. Although the nurses do not have enough equipment to ensure the intimacy of a patient in the postoperative care, they know how to take care of intimacy in the postoperative care.
Human dignity and nurse
HRŮZOVÁ, Marcela
The aim of the thesis is to find out the conception of human dignity at medical staff. At nurses, in addition, with a hidden observation to verify to what extent they meet their statements in practice, whether for nurses a patient does not become only an object of relevant therapeutic and nursing acts. The empirical part of this thesis was processed in form of a qualitative research. The data collection technique was a non-standardised interview with nurses at standard in-patient departments, students of the nursery Master study programme, students of a higher nursing school and patients at a standard department. At nurses it was verified, using the technique of hidden observation, to what extent their actual behaviour in practice corresponds to the information mentioned in interviews. Therefore in their interviews we differentiated the mentioned and non-mentioned elements. The interviews were anonymous. They were recorded in writing because only in one case the respondent agreed for us to use a dictaphone. Collection of empirical data proceeded from June to August 2008. Unlike our expectations the research showed that respondents have no conception of human honour and have no complex conception of its keeping in practice. We categorised the rather poor information into five basic categories that covered 92% (intimacy, communication, individuality, empathy and equality of approach). Statements that we did not manage to categorise here create other 5 categories (worthy dying, consider seriously, pain calming, lege artis process and faith). Most often the respondents mentioned the following categories: intimacy (80.4 %), communication (76.8 %), individuality (60.7 %), empathy (39.3 %) and equality of approach (33.9 %). The hidden observation showed that most nurses (13 out of 16, i.e. 81 %) adhered to their fundamentals of keeping human dignity when working with ill people in a satisfactory way. The other 3 nurses made mistakes in all categories except keeping intimacy; here only one of them did wrong. Results of the thesis should contribute to improvement of quality of care and help to better respect for human dignity.
Nursing care about colostomies in unsurgical nursing units
DOČEKAL, Adam
My Bachelor Paper has surveyed the professional skills of nurses staffing a non-surgery ward. The survey covered nurses working at the Internal Medicine Wards of the České Budějovice and Jihlava hospitals. I have chosen this subject prompted by my personal experience which leads me to believe that the nurses on the staff of this department are not properly trained, neither in theory nor in practice, to treat colostomy patients. The Czech Republic is a home to about eight thousand patients with this condition, people of different ages and occupations, living active and fully independent lives. As these patients can be hospitalized also in the IM Ward, it comes as a matter of importance that the on-site nurses possess the required know-how so that they do not subject the patients / clients to avoidable stress.
Observing patients' rights in selected hospitals of the Czech Republic.
KORTUSOVÁ, Jana
For everyone hospitalization represents a change accompanied by various restrictions. Anyway, this cannot mean a change in rights. Each patient is unique {--} they have specific needs, unchangeable principles, a value system of their own. A patient can be defined as a person under medical care, as an ill person undergoing treatment. The term {\clq}qpatient is derived from the Latin word {\clq}qpatiens`` {--} patient, suffering. The patients' rights are laid down in the Code of Patients' Rights which became effective in the Czech Republic in 1992. It is the knowledge of rights that is important before all for the strengthening of an equal relationship between the doctor and the patient The risks which represent a threat during the hospitalization can be lowered by consistent observation of the patients' rights as well as the patient's satisfaction with the care provided can be heightened. The basic individual rights of the patients include the right to silence to be preserved about the facts that the health worker became familiar with in connection with medical care or treatment, the right to being informed about their state of health, the right to gaining necessary information in order to be able to make a decision before a new diagnostic or therapeutical procedure is started up. The other rights include refusing a treatment and being informed about the consequences of the decision, the right to privacy during medical care or treatment and to a considerate, sensitive and worthy approach during the treatment. The thesis comprises the present state of problems in question, the aims of the thesis, determination of hypotheses, methodology, the results of the research, discussion, conclusion, the list of sources used, key words and supplements. The present state portrays the history of ethical codes, legal standards and the problems of medical law, the patients' rights, individual patients' rights, accreditation standards of the patients' rights. The aim of the thesis is to find out whether the patients are made acquainted with their rights after being taken to hospital, whether they are informed about the character of the disease, about suggested treatment and possible treatment alternatives. To map out whether there is the need of education being taken into account with each patient and whether the nurses keep notes in the documentation and find out the nurses' knowledge in the sphere of the patient's immobility. here have 7 hypotheses and 5 statistical hypotheses been specified in the thesis. Quantitative research approach was applied for the research. Questioning was used as a method, a questionnaire as a technique. The questionnaire for the patients was anonymous and included 26 questions. It focused on making the patient acquainted with their rights, on awareness of the state of health, the character of the disease and the suggested treatment, on the education of the patient by the nurse and on respecting the patient's intimacy. It included closed and semi-closed questions. At the end of the questionnaire the patients had a chance to express their possible comments, suggestions, impulses concerning the problems in question. The questionnaire for the nurses was anonymous and included 32 questions. It had its focus on observing the patients' rights, the informed patients' consent, their education, on the problems of restricting the patient on the bed and on respecting the patient's intimacy. The questionnaire included closed and semi-closed questions. The representative sample was made up of 218 patients and 205 nurses. The results were compiled in tabular and chart form, and subsequently processed by means of the SPSS statistical programme. The discussion includes a summary of the research knowledge in comparison with the sources used.
