Development of population ageing and its impact on long-term care expenditures in the Czech Republic

This paper examines and analyzes development of population ageing in the Czech Republic and its likely future development and impact on long-term care costs. Long-term care is defined in detail and subdivided. Views of the European Union and the Czech Republic on the future development of long-term care are also summarized. Selected categories of expenditures are described and analyzed, including differences in spending on inpatient care and home care. The model, which tests whether and how population aging and other factors (e.g. GDP, number of beds, etc.) affect the long-term care expenses is processed at the end of the thesis.
Rehabilitation Nursing in CVE Patients in Home Care

The research reveals correlations between nursing quality and additional education of HCA nurses.
Dilemmas linked to the future of the affected siblings

This thesis focuses on the dilemmas linked to the future of the affected sibling and the ensuing arguments, which in the future may help to siblings without disabilities in their decisions on the future sibling with a disability. The introduction defines basic fundamental concepts: combined disability, mental retardation and associated with the most common combinations of disability. Subsequently, the work is focused on the complexity and pitfalls associated with caring for people with multiple disabilities. On the sibling relationship, which can have a big impact on the future of siblings with disabilities, but also on the possible reasons causing disability and dilemmas associated with it based on the future care of siblings with disabilities. Furthermore dissertation discusses the possible variants of care that might siblings without disabilities to facilitate their decision-making and assist in the eventual home care. In the next part closer look at the particular dilemmas, with whom, according to interviews siblings without disabilities might face in the future. Individual dilemmas was determined on the basis of the research probe that howled implemented method of qualitative research through interviews with siblings of people with multiple disabilities. Towards the end of the thesis is to analyze the chosen dilemma, which is subject to the conclusion obsáhlému analysis from the perspective of two ethical theories - utilitarianism and deontology.
Nursing documentation by the home nursing care

Nursing documentation is currently still very hot topic. Perhaps because they are still many health workers (physicians and non-physicians) sufficiently understand the importance and seriousness of this document. The documentation not only provides valid information about the client and his health, but also ensures continuous flow its nursing care and ultimately became a piece of evidence in case of litigation. Documentation can also be used for education and research purposes. Home care is a field that has recently rapidly growing and gaining in popularity. As part of primary and community care nursing provides clients in its natural, familiar and domestic environment, which is a big advantage for him, he does not spend his days in hospital, which is economically difficult anyway. Home Care provides health care services indicated by the treating physician or practitioner and are paid for by health insurance. This thesis deals with the nursing documentation in home care agencies, which aims to describe the demographic impacts on the use of nursing documentation, check the attitude of nurses from selected agencies of home care to nursing documentation and analyze the nursing documentation in selected home care agencies. The thesis is composed of two parts, theoretical and empirical. The theoretical part deals with the history of home care, statistical data shows the growth of agencies, their employees and clients, providing comprehensive home care and medical documentation. The empirical part was realized in the form of qualitative research. The information was gathered in-depth interview technique, which was compiled from 1 identification questions and 11 open-ended questions that formed the framework for the entire interview. In fact, respondents to individually comment on the issue to the extent that they saw fit. The research group consisted of seven sisters of different age, education and length of experience, currently working from home care. Due to fears nurses were promised total anonymity, so you can just tell that they were sisters from ADP capital city of Prague and Central Bohemia. Complementary research was conducted in seven home care agencies for the same regions, but any of nurses does not belong to the Agency from the second survey. In the first phase of the research we examined the influence of nurses' demographic factors such as age, education or length of experience in the management of nursing documentation and own attitudes of nurses from selected agencies in the management of nursing documentation. The research sample consisted of seven nurses working in home care agencies that were selected purposefully and were willing to cooperate in research. In the second phase of the research we tried to analyze individual nursing documentation of selected home care agencies. For this part, we chose seven agencies, which have been their leadership under the promise of anonymity willing to cooperate. The research investigation set up three goals and three research questions. The first objective was to describe the demographic impacts on the use of nursing documentation. The second objective was to determine the attitude of nurses from selected agencies of home care to nursing documentation. The third objective was to analyze the nursing documentation in selected home care agencies. At the outset of the investigation, we asked the following questions: they influence the management of the nursing documentation demographic factors such as age, education or length of practice nurses? What attitudes toward nursing documentation occupy the sisters from selected home care agencies? Correspond to different parts of the nursing documentation with the phases of the nursing process? Results of the research can be an inspiration not only for the nursing management ADP, but also for the nurses working in home care agencies throughout the Czech Republic, only selected agencies.
Effectiveness of cooperation of family and home care agency in nurse care of senior citizens during the period immediately after discharge from hospital

I wanted to support and make visible the importance of providing the Czech Red Cross home care service in Strakonice concerning senior care in their own houses. The theoretical part deals with senior person, family, and home care characteristic. The survey part is processed by means of qualitative research methodology, where inquirers were nurses of the home care and clients with families, who used their service. Objective of this thesis was to find out, if the family members use the home care service in the process of senior care and which area their cooperation consists in. The outcomes are processed using interviews with the nurses of the home care and with the family members and seniors. Main research questions were: What is the way the patient and his/her family are informed about opportunities of using home care service when discharging from hospital? To what extent do family members use the service of home care? Do family members miss some particular activity beyond contemporary provided services? How does the family cooperate with the home care nurses in care of the senior after his/her discharge from hospital treatment? What activity do the family members need to be done by the home care nurses in senior care after patient's discharge from hospital treatment? On the basis of these questions we were creating sub-questions to the interviews with home care nurses and families with clients. Nurses were answering 16 questions and the family with seniors 13 questions. This thesis is written for conclusiveness of the importance and the irreplaceable of the home care in the process of treatment of the senior in his/her natural environment. The result of the thesis points to contentment and indispensability of provided service by the client.
Treatment of a dependent geriatric pacient in domestic environment

The issue of the treatment of geriatric patients in a home environment is due to the more current demographic trends. The treatment of these patients is not easy and brings a number of problematic situations. Therefore, it is necessary to think, what the quality of care is in a home environment and how the family of geriatric patients on changes in his health will adapt. It is also important the education and the support of the lay family caregivers in the theoretical and practical knowledge and skills necessary to provide high-quality care. In the theoretical part of the outlined the division of age, characteristics of the geriatric patient, problems that can affect the patients and the carings and the home care. For geriatric patients are mainly the geriatric syndromes and the geriatric frailty. The carings and health professionals talk often about burnout. The main objective of the Bachelor thesis was to map the treatment of the infirm geriatric patients in a home environment. The singular targets have been set as follows: The experience of the non professional caregivers care of geriatric patients infirm in the domestic environment. To find out the problem situation in connection with the care of geriatric patients infirm in the domestic environment. Find out ways to resolve problem situations in the care of geriatric patients infirm in the domestic environment. To find out the needs of non professional carers in care of geriatric infirm patients in a home environment. To find out, how is the cooperation between doctors, nurses and lay carers to ensure the care of geriatric patients in a home environment. Research file was consisted of the geriatric infirm patients older 65 years in home care of the non professional lay carers, their family members and the professional nurses of the home care agencies. For the Bachelor thesis was used qualitative research strategy. To obtain the requisite information has been used polling method, technique of data collection was a free interview. The obtained data was processed by the technique of open coding, method of pencil and paper. Subsequently, the reduction was carried out with the data organized into categories and subcategories. There were created seven categories and sixteen subcategories. Some caregivers suffer from economic, social, psychological and health problems. It was found that the caregivers were forced to do in their lives many fundamental changes. They all had to adjust the housing, in some cases they had to move or even leave their jobs. Furthermore, it was found that the family is not whole in all patients, and therefore in two cases, there is nobody who could who rotates in the care of the infirm member of the family. Many of the problems were turned out as well for example, that many patients suffer from the diaper rashes, problems with defecation, obesity, depression, and other. Lay caregivers do not have enough information about the care or compensatory utilities. Often they suffer from health problems, such as back pain, depression, burnout, insomnia and other. The work can be used as a source of information for students of nursing and professional nurses. Many of the shortcomings are evident after charting nursing home care. The most alarming is the lack of information. Therefore it is necessary to develop care and ensure that most patients could be treated at home. The economic and psychological support of the non professional caregivers and especially the prevention of burnout are highly important. Home care can lead to the better medical outcomes and the lower costs for care.
