Social Worker´s Role in a Comprehensive Care for Autistic People

The bachelor´s thesis deals with the role of a social worker in a complex care of autistic people. The main goal of the thesis is to describe some methods, techniques and attitudes used in a context of social work and the role of a social worker. The intermediate objective is to map some worker´s experience concerning the complex care of autistic people. The role of a social worker within the framework of a multidisciplinary team, some methods, techniques and attitudes used in a complex care of autistic people have been found via the research questions. The bachelor´s thesis is divided into a theoretical and a practical part. There are defined some basic terms: autism, social worker, social work and a multidisciplinary team in the theoretical part. Autism and its diagnostics is more specified for better understanding. That gives us opportunity to familiarise with the contributory organisation Domov Kopretina Černovice where the research strategy is placed. There are also introduced and described some techniques and methods used in a complex care of autistic people in the theoretical part. The practical part consists of a research using a qualitative research strategy, methods of questioning and semi-structured interview techniques. The research group consists of ten communicative partners who belong to the multidisciplinary team taking care of the autistic children in the contributory organisation Domov Kopretina, Černovice. Within the framework of the research we´ll learn about an educational programme in a special primary school and practical school applying in Domov Kopretina, Černovice. The research has shown that the job description of a social worker consists of social agenda tackling in Domov Kopretina, Černovice. Also the other members of the multidisciplinary team see the role of a social worker in the administrative sphere instead of immediate social care of autistic people. The social worker is also regarded as an important member of the team because he/she has the earliest opportunity to get in touch with the client and consequential coordinated rehabilitation that means the acceptance the client to join the organisation. Title: Rehabilitation of patients with spastic form of Infantile Cerebral Palsy from the perspective of physiotherapy Abstract: The thesis deals with the rehabilitation of persons with cerebral palsy, physiotherapy intervention for appropriate regular exercise and the impact on quality of life of people with cerebral palsy. The theoretical part describes the introduction to the issue - the definition of the correct terminology, and problems of cerebral palsy - its characteristics, prevalence, classification, diagnosis, etiology and appropriate intervention. The practical part is focused on physiotherapy intervention. The aim of the thesis is to determine the impact of selected interventions on quality of life of people with cerebral palsy. Coordinated Rehabilitation for Recurrent Respiratory Diseases of Children

Respiratory diseases are ones of the most common diseases, which are treated by general practitioners. Not only the right diagnosis is essential, but the treatment as well. The treatment of respiratory diseases is usually long-term and often requires repeating. Health treatment of young children in a spa improves their life and health quality, inclusion in a collective of similar aged children, reduces complications of their disease, doses of medicine and highly reduces the risk of development of a chronical disease in adulthood. The aim of this thesis is to chart the use of coordinated rehabilitation in spa Lázně Luhačovice, a. s. and to survey coordinated rehabilitation awareness of spa medical and non medical personnel and children's accompaniment. Three hypotheses were set. Hypothesis 1: Children's accompaniment, who has been to spa health resort repeatedly percieves the care of his child as coordinated unlike the one who has been to spa health resort just once. Hypothesis 2: Medical staff of children spa health resorts percieves the most the medical treatment part of coordinated rehabilitation in contrast with non medical staff. Hypothesis 3: Children's accompaniment is satisfied the most with the medical treatment part of coordinated rehabilitation unlike the pedagogical and social one. A strategy of quantitative research was used, data was gathered by standardized questionnaires in June and July 2015. The standardized questionnaires were given to two different research groups. First one was spa health resort personnel, second one children's accompaniment in the spa Lázně Luhačovice, a. s. Based on statistical calculations the hypotheses can not be rejected, the results are not statistically significant. From the data obtained from the questionnaires, it is noticable that from the spa health resort medical and non medical personnel point of view, there is percieved only the part, which they provide. On the other hand, from the children's accompaniment point of view the rehabilitation is percieved as coordinated in all the parts provided. It is thanks to the fact that the accompaniment, who is in the spa health resort on the expenses of health insurance company, can see all the parts of coordinated rehabilitation as they undergo it together with their children. This thesis contributes to widening awareness of using parts of coordinated rehabilitation in children repeated respiratory diseases treatment, which are provided in facilities of spa Lázně Luhačovice, a.s. and in other health resorts, where there is this kind of care provided as well.
The experiences of adults with disabilities with the provision of coordinated rehabilitation in the institutionally care and in their natural environment.

The diploma thesis deals with the issue of providing all elements of the comprehensive rehabilitation of handicapped people. The goal of this thesis was to determine, what experiences adult handicapped people have in terms of being taken care of in their home environment and in an institution. Another goal was to try and find out what criteria are important for the way they evaluate these. And, the last goal of the thesis was to propose an effective model of taking care of these people given obtained results and given financial capacities of the state and today's regulations. The theoretical part shows the section and development of care provided to handicapped people from the beginning of humanity until today including valid legal regulations. The following part describes briefly the issue of a coordinated rehabilitation and the efforts to ensure the legal regulation thereof in the Czech Republic. The last chapter of the theoretical part is devoted to the D.R.A.K. Association, the clients of which are the relevant research set. For the research itself the qualitative research strategy was chosen. The data were acquired using semi-structured interviews with clients, their family members and those who care for them in the D. R. A. K. Association. Acquired data were processed using the qualitative contents analysis of interviews. The set of participants included nine clients, members of the D.R.A.K. Association, the target group of which are adult handicapped people. Next, I spoke with five family members and three experts from D. R. A. K. Results are divided into three chapters according to research questions. The first chapter contains answers of clients, how they perceive the difference when being taken care of in an institution (D.R.A.K. Association) and in the natural environment (at home). The second chapter of results describes criteria, which are decisive and important for clients and have emerged during interviews.
The Family and the Counselling in Rehabilitation

The thesis deals with the role of family in the course of coordinated rehabilitation. At present time, rehabilitation is understood not only as a medical care within the sphere of physiotherapy but also as an interdisciplinary activity which includes health-care, social-legal, pedagogical-psychological as well as work-related activities. Coordinated rehabilitation therefore requires cooperation of a team of involved specialists. This is not only about qualified experts but also about the disables person and his/her family. Family is the most important social group and the basic building stone of society. It fulfils the basic educational function, forms the attitudes of the child to itself, its loved ones and the whole world. Thus it fundamentally affects the society and on the contrary the society influences the family. That is why the family has to receive information necessary for the fulfilment of its functions (in particular when taking care of a disabled person), for example in the form of expert counselling. The objective of the work was to identify the role of family and its cooperation with counselling agencies within the process of coordinated rehabilitation. In addition to agencies taking care of handicapped persons, counselling institutions include also organisations whose social services are primarily provided to this target group. Qualitative research was used in the course of work on the empirical part. Input data were collected by the method of questioning, in particular by the technique of semi-structured interview. The research group consisted of thirteen social workers who provide expert social counselling or other social services primarily intended for handicapped persons. In connection with the objective of the thesis, the main research question was: What difficulties social workers identify when working with the family within the process of coordinated rehabilitation? The sub-question derived from the main research question was: What is the role of family within the counselling process from the perspective of a social worker? The results showed that social workers are satisfied with the families of users of the social service and confirm that cooperation between them is good. However, cooperation between social workers and the family differs significantly according to the type of provided services. The research also revealed that providers of social services in the course of their work with family encounter difficulties rather exceptionally. Still, social workers perceive as difficulties especially limits in the communication with family. Providers of social services try to build relations between them and the family of the user of the particular service and provide social support. Social workers perceive the role of family especially as being their communicating and cooperating partner. The research showed that the best cooperation and communication between social workers and the family of user of the social services is one based on a partnership level. The thesis identified the difficulties of social workers encountered during their work with families taking care of a handicapped person and suggested recommendations for more active involvement of families in dealing with a difficult life situation of its members. In addition, the thesis can serve as a basis for further quantitative research of this theme.
Coordinated rehabilitation segments used in a selected group of individuals with DMO (combined disability) in center ARPIDA in České Budějovice

The thesis focuses on coordinated rehabilitation components, which are used at individuals with cerebral palsy (combined disability) in a concrete non state medical institution in České Budějovice. Cerebral palsy is the most frequent neurodevelopmental disease, wherein the health disability may occur in one or more fields. We can find crucial factor in care of individuals with handicap in coordinated rehabilitation, which is currently perceived as an interdisciplinary field seeking the fastest and widest participation of personal with disabilities in all usual activies of society with the use of medical, educational, social and labour resources. A prerequisite for success is a multidisciplinary team (circular support), which consists of experts and parents of individuals with disabilities. The thesis contains theoretical background, where health disbility, coordinated rehabilitation and cerebral palsy is mentioned, at which the occurrence, forms, associated disorders and treatment are described. A separate chapter is devoted to families of individuals with health disability, where I mentioned the reaction of parents on the birth of a child with a disability, the impact of disability on the family and family support. Furthermore I focus on social services for handicapped people and to ARPIDA centre, where my research took place. In the research section there is defined the aim of the thesis, which was to chart a coordinated rehabilitation components, which are used at individuals with cerebral palsy (combined disability) in ARPIDA centre in České Budějovice. The partial target of my thesis was also to compare the usage of coordinated rehabilitation components at individuals under 15 years of age and at individuals over 15 years. According to target of the work there were developed three hypotheses. Thesis research was conducted through quantitative research. To obtain the data, I used the method of questioning. The sample was comprised of individuals with cerebral palsy (combined disability), who attend the centre ARPIDA, while the questionnaires were filled out by their parents. The research undertaken within the thesis, which was supported by rehearsed literature, confirmed the third hypothesis of more often usuage of Vojta reflex locomotion method at individuals with cerebral palsy in the centre ARPIDA compared to individual therapeutic physical training. The remaining two hypotheses were disproved. Statistical survey also shows that care for people with cerebral palsy is a long-term care, where interconnection of services within the coordinated rehabilitation is needed. In the group of individuals under 15 years of age and over 15 years it has been used different coordinated rehabilitation components, this is associated with cerebral palsy, disorders, age and other factors. Problematic remains the job placement of individuals with disabilities, who remain at home without further stimulation possibility. Care and services in the centre ARPIDA were assessed positively. The results and conclusions of the thesis will be used as a feedback centre ARPIDA to map the coordinated rehabilitation components and further to the clearing of needs of single coordinated rehabilitation components in a group of individuals under 15 years of age and over 15 years.
Coordinated rehabilitation for clients of Institution of social care Domečky

The diploma thesis deals with the coordinated rehabilitation. Coordinated rehabilitation is a coherent process of medical, educational, vocational and social rehabilitation, whose main objective is to minimalize the consequences, based on the individual's disability. Successful rehabilitation consists in ensuring early, continuous and coordinated efforts of the fastest and widest participation of persons with disabilities in the usual activities of life.The theoretical part consists of six basic chapters. The first one is devoted to the International Classification of Functioning, Disability and Health, which is followed by the subchapter dealing with disabilities and consequently mental disabilities. I have focused on people with mental disabilities, as an organization where research was conducted, providing services to these people. The second chapter contains information about coordinated rehabilitation. The remaining chapters deal with the individual components of a coordinated rehabilitation, which are medical, educational, vocational and social rehabilitation. The aims of this thesis is to determine how the system of coordinated rehabilitation is used in the Institute of social care Domečky. A partial aim is to determine the importance of a system of coordinated rehabilitation for clients of the Institute of social care Domečky. There are two research questions in the connection with the thesis aims. The main research question is: How are the individual components of a coordinated rehabilitation applied in the Institute of social care Domečky? The particular research question finds positives provides system of coordinated rehabilitation for clients of the Institute of social care Domečky.Qualitative research was chosen for the practical section of the thesis. I have used the questioning method and the technique of the semi-controlled dialoque, Results were evaluated using the method of make the clusters. The selected group consisted of 11 employees of the Institute of social care Domečky. The respondents have been chosen intentionally, according to the criterion of willingness to participate in research. The results are divided into two sections according to the research questions. The first section deals with the way the use of individual components of the coordinated rehabilitation in the Institute of social care Domečky. These results are also divided by the individual components of coordinated rehabilitation for greater clarity. The first part contains the identification of the respondents. Following parts: medical rehabilitation, educational rehabilitation, vocational rehabilitation and social rehabilitation.The second part of results includes the subjective opinions of the respondents on the benefits of coordinated rehabilitation and its benefit to the clients of the organization.The research has shown ignorance of the concept of coordinated rehabilitation. Within medical rehabilitation it was found that health care is provided by nurses and practical doctor. The organization also works with hospitals and polyclinics, which are next to the organization. The research, which is devoted to educational rehabilitation, results divided terminology used in special education. Vocational rehabilitation can be used by clients within a job in the kitchen and the laundry in the organization or employment in the civic association Pferda which offer a job in a cafe, a cleaning company and a bakery.Within the social rehabilitation it was described the relationship of key worker and client and the creation of individual planning. It was clarified the issue of social security benefits when clients receive invalidity pensions, care allowance, alternatively child benefit. From the perspective of respondents the benefit of coordinated rehabilitation is in complex care for clients that is devoted them.
The comprehensive rehabilitation methods on the mentally disabled in south bohemia.

The comprehensive rehabilitation provides complex care to disabled individuals using all types of therapeutic approaches that are part of the comprehensive rehabilitation. These are medical, educational, social and occupational approaches and the interconnection of all four of them is both vital and preferred. The comprehensive rehabilitation is beneficial for people with mental as well as physical and multiple disabilities. The use of the therapeutic approaches can be tailored to suit the individual needs of the disabled patients. The present bachelor thesis studies the comprehensive rehabilitation of individuals with mental disabilities. The theoretical part describes the coordinated rehabilitation as presented by various authors dealing with this topic. The first chapter defines the term "coordinated rehabilitation", the second explains the term "mental disability". The definition of mental disability also introduces the degrees of the disability, which include mild, moderate, severe and profound mental retardation. Basic definitions and information are given for all the above degrees. Chapters 3 6 describe the educational, occupational, medical and social therapeutic approaches of the comprehensive or coordinated rehabilitation, studying each approach in detail. Chapters 7 9 present South Bohemian rehabilitation facilities, especially those in the town of Ceske Budejovice namely ARPIDA, Bazalka, o.p.s. and Domov Libníč a Centrum sociálních služeb Empatie, describing all services provided by these facilities. All the facilities provide specialized care for people with mental, physical and multiple disabilities. The thesis aims to determine whether South Bohemian rehabilitation facilities make sufficient use of the comprehensive rehabilitation approaches and, in general, of all available resources in providing care to people with mental disabilities. The practical part deals with the research into methods used in the comprehensive rehabilitation facilities in South Bohemia, chiefly in ARPIDA, Bazalka, o.p.s. and Domov Libníč a Centrum sociálních služeb Empatie. The practical part relies on a qualitative research conducted through semi-structured interviews. This research involved 6 respondents from ARPIDA, Bazalka, o.p.s. and Domov Libníč a Centrum sociálních služeb Empatie interviewed about the comprehensive rehabilitation methods to treat mentally disabled patients used in their facilities. A total of 10 pre-defined questions were used in the interview (the questions are given in the Annex to this thesis). Two respondents from each facility willing to participate in the research were chosen for the interviews. With their consent, their responses were recorded and the content of the responses was then transcribed. The results of the research imply that South Bohemian rehabilitation facilities, especially those of ARPIDA, Bazalka, o.p.s. and Domov Libníč a Centrum sociálních služeb Empatie, make sufficient use of all comprehensive rehabilitation therapeutic approaches to treat people with mental disabilities, with quality care provided by qualified personnel. Another conclusion shown by the research results is that the rehabilitation of mentally disabled patients relies heavily on occupational, social and educational therapeutic approaches, leaving medical rehabilitation slightly less accented. Medical therapeutic approach is used mainly to treat mentally disabled patients with associated physical disabilities or for individuals where medical therapy was prescribed by a physician. The thesis could serve as a resource for students dealing with comprehensive rehabilitation or with mentally disabled individuals. Furthermore, the thesis may help people with mental disabilities and their families in choosing a suitable rehabilitation facility in South Bohemia as it provides information on the facilities, on the services they provide and on the comprehensive rehabilitation therapeutic approaches used.
Methods of coordinated rehabilitation provided to individuals with physical disabilities

This bachelor thesis called Methods of coordinated rehabilitation provided to individuals with physical disabilities focuses on school-age children. The thesis is divided into two parts. The theoretical part deals with the system of coordinated rehabilitation, its development and meaning. Means of rehabilitation namely remedies, educational, social and working resources with regard to their interconnection and mutual continuity are closely defined in particular subcategories. Therapeutic methods are closely specified in the subcategory regarding rehabilitation remedies. The subcategory concerning social rehabilitation resources concentrates on social services. Second chapter pinpoints physical disability, its etymology and classification. Specific types of physical disability are cerebral palsy and neuromuscular disease. The last chapter of theoretical part is devoted to methods of coordinated rehabilitation that can be used for children with physical disabilities. The research took place in Primary school and nursery school and provider of social services, Kaňka social purpose society in Tábor. The aim of the practical part was to find out the methods that this centre under the system of coordinated rehabilitation provides to its clients. Another aim was to point out whether this system works on case reports of chosen children in such facilities. Qualitative research has been chosen to fulfil the aim of this thesis. The data has been obtained primarily by the content analysis of the data. Semi structured interview with parents served as an additional method. On the basis of the content analysis of the documents three case reports of children with physical disabilities have been created. Informed consent has been acquired from the children's parents. Case reports reflect personal and family history, information in medical reports, psychological examinations and individual plans. Furthermore, attention is paid to particular means of coordinated rehabilitation that are used by the children in Kaňka centre. The research showed that notably thanks to teamwork Kaňka centre works on the basis of comprehensive (coordinated) system of rehabilitation. It is apparent from case reports that continuity of remedies, educational and social resources is good. Working resources are not crucial for this work because attention was paid on school-age children. On the other hand, Kaňka centre provides occupational therapy especially for children with physical disabilities. Occupational therapy represents a practise of daily activities and development of self-sufficiency, which closely relates to working resources of coordinated rehabilitation. Facility interconnects specialist therapies with social services and compulsory school attendance. Children are provided these therapies: physiotherapy, speech therapy, canine therapy, music therapy, art therapy and occupational therapy. As far as the children with physical disabilities are concerned they practise ordinary daily activities, develop fine and gross motor skills, improve mental health and overall wellbeing and promote a positive attitude. In terms of social resources these services are used: personal assistance, day care centre and early care. Children are educated in primary school and special school for primary education. Kaňka also provides special care nursery. Children are also offered leisure activities. Primary school and nursery school and provider of social services, Kaňka social purpose society is one of few centres in southern Bohemia region that provides complex care in the spirit of coordinated rehabilitation that is not only for children with physical disabilities.
Comprehensive system of rehabilitation in younger individuals after stroke

Title of the bachelor thesis is Comprehensive rehabilitation in younger individuals after stroke. The aim of this thesis was to determine the subjective perception of individuals´ needs after stroke. The theoretic part is divided into two main chapters. The first one contains brief information about the disease, its types, anatomy and physiology background and symptoms. The second chapter deals with the issue of comprehensive rehabilitation and its parts. Particular approaches are defined and described in detail, health professionals working in this area are also mentioned here. The research part was processed by a method of qualitative research strategy. Semi-structured interview technique and observation were used. The research set included 5 respondents - four men and one woman. Past history of stroke and age of individuals between 45 and 55 years were the inclusion criteria for this research. Interview was held according to scenario questions, where I focused on two areas. First section included information about the respondent, his disease and treatment in the way of comprehensive rehabilitation. In the second part, respondents were asked how their life had changed after the experience of stroke. The interview was audio-recorded and then verbatim transcribed. One of the respondents did not wish to be audio-recorded, thus written notes were taken during the interview. Interviewing results are presented in a narrative format. By conducting this research I tried to answer the following question: What are the needs of individuals at various stages of stroke recovery? Need of self-sufficiency was mentioned by all respondents. In early stage, it was mainly coping with basic activities such as food intake, control of excretion, verbal expression and movement. In the later phase of treatment, there are increasing demands on self-care, understanding ability, memory and social interaction. Individuals often went through an intensive therapy. After returning back to their home environment, the need of self-realization, activation and meaningful ways to spend a free time are the most significant issues. This work should provide more information on the given topic.
