[cze][eng] Ústředním předmětem porodnictví je žena a plod - novorozenec. Úkolem všech, kteří se o ženu a dítě starají, je péče o zdraví fyzické, ale také o psycho-sociální stránku. Péče by měla být holistická, komplexní a individuální se zachováním kontinuity a toto vše splňuje komunitní péče porodní asistentky. Z teoretických podkladů světových i českých by se dalo předpokládat, že komunitní péče porodní asistentky má důležitou úlohu v péči o ženu a dítě. V praxi však porodní asistentky narážejí na překážky. Hlavním cílem této práce bylo zjistit zájem žen o péči komunitní porodní asistentky v oblasti domažlického okresu. Dalším cílem je zjištění názoru klientek, které využily služeb terénné porodní asistentky. Dále názor komunitních porodních asistentek na potřebu této péče a také zájem praktických ženských lékařů ke spolupráci s terénní porodní asistentkou. Pro zpracování dané problematiky bylo zvoleno kvantitativní i kvalitativní výzkumného šetření. Hypotéza 1 ‚qŽeny v oblasti domažlického okresu mají zájem o komunitní péči porodní asistentky v těhotenství“ byla potvrzena. Hypotéza 2 ‚qŽeny v oblasti domažlického okresu mají zájem o komunitní péči porodní asistentky po porodu“ se také potvrdila. Hypotéza 3 ‚qŽeny v oblasti domažlického okresu uvádí alespoň 3 konkrétní oblasti, ve kterých by využily péči porodní asistentky v terénu“ byla rovněž potvrzena.. Na výzkumnou otázku 1 ‚qPovažují ženy, které využily služeb komunitní péče porodní asistentky tuto péči za potřebnou?“ odpověděly všechny respondentky jednoznačně pozitivně. Na výzkumnou otázku 2 ‚qPovažují porodní asistentky, které mají zkušenost s prací v komunitní péči, tuto péči za potřebnou?“ odpověděly porodní asistentky pozitivně. Na výzkumnou otázku 3 ‚qV jakých oblastech péče o ženu vidí praktičtí ženští lékaři možnost spolupráce s porodní asistentkou v terénu?“ odpověděli praktičtí ženští lékaři, že vidí možnost spolupráce s porodní asistentkou v terénu především v doporučování předporodní přípravy, u návštěvní a prenatální péče je spolupráce omezená.’q Práce bude dále využita pro podporu rozvoje komunitní péče porodní asistentky v České Republice.The central subjects of midwifery are a woman and a fetus - a new-born child. The task of taking care of a woman and her child is to look after not only their physical health but also to look after them in sense of the psycho-social sphere. The care should be holistic, complex and individual, preserving its continuity. All this fulfils the midwife’s community care. Going from the theoretic world and also Czech materials we could suppose that midwife’s community care plays an important role in woman’s and child‘s care. Practically however, midwives meet obstacles. The main aim of this thesis was to find out women’s interest in midwife’s community care within the district of Domažlice. The next aim was to find out the opinion of those women who made use of a midwife’s fieldwork. The further aim was to find out the opinion of the community midwives’ requirement of this care and also the interest of gynaecologists-practitioners in cooperation with a midwife working in the field. For the processing of the problem given, both quantitative and qualitative research was used. Hypothesis 1 “Women in the district of Domažlice are interested in midwife’s community care in pregnancy’q was affirmed. The women were mostly interested in antenatal preparation and pregnancy exercises. Hypothesis 2 “Women in the district of Domažlice are interested in midwife community care after childbirth’q was affirmed. The interviewees were mostly interested in the visiting service, breast-feeding consultations. Hypothesis 3 “Women in the district of Domažlice mention at least 3 specific spheres that they would use the care of a midwife in the field’q, was affirmed. Mostly they were interested in the visiting service, antenatal preparation and pregnancy exercises. The research question 1 “Are the midwife’s community care services regarded as useful by the women who used it?’q was answered explicitly positively by all of the interviewees. The research question 2 “Is the community care regarded as useful by midwives who have experience with it?’q Was answered positively by 5 out of 6 midwives. To the research question 3 “What spheres of woman’s care do the gynaecologists-practitioners regard as a possibility of cooperation with a midwife in the field?’q The practitioners explicitly expressed themselves for psycho prophylaxis and pregnancy exercises. This thesis is intended for a further support of development of the community midwife’s care in the Czech Republic.
