Tato bakalářská práce se zabývá významem techniky hlubokého nádechu (Deep Inspiration Breath Hold - DIBH) při radioterapii karcinomu prsu. Cílem práce je zhodnotit přínos této techniky z hlediska snížení dávkového zatížení kritických orgánů, zejména srdce a levé přední sestupné koronární tepny (LAD), a porovnat tyto hodnoty s ozářením ve volném dýchání. Teoretická část shrnuje anatomii prsu, charakteristiku karcinomu prsu, možnosti diagnostiky a léčby, a podrobně se věnuje principům radioterapie a technice DIBH. Praktická část je zaměřena na analýzu plánů ozařování u vybraného souboru pacientek, u nichž byly porovnávány střední dávky na srdce a LAD ve volném dýchání a při použití techniky DIBH. Výsledky ukazují, že technika DIBH vede k významnému snížení dávkového zatížení těchto struktur, a potvrzují tak její přínos pro minimalizaci rizika pozdní kardiotoxicity. Práce zdůrazňuje důležitost správného použití techniky DIBH v klinické praxi a její potenciál ke zvýšení bezpečnosti radioterapie u pacientek s levostranným karcinomem prsu. This bachelor's thesis is focused on the significance of the Deep Inspiration Breath Hold (DIBH) technique in radiotherapy of breast cancer. The aim of the thesis is to evaluate the benefits of this technique in terms of reducing radiation dose to organs at risk, particularly the heart and left anterior descending coronary artery (LAD), and to compare these values with the dose received during free breathing irradiation. The theoretical part summarizes the anatomy of the breast, the characteristics of breast cancer, and the possibilities of diagnosis and treatment. It also provides a detailed description of the principles of radiotherapy and the DIBH technique. The practical part focuses on the analysis of radiotherapy treatment plans in a selected group of patients, in which the mean doses to heart and LAD were compared during free breathing and when using the DIBH technique. The results show that the DIBH technique leads to a significant reduction in radiation dose to these structures, confirming the benefits in minimizing the risk of late cardiotoxicity. The thesis also emphasizes the importance of the correct implementation of the DIBH technique in clinical practice and its potential to increase the safety of radiotherapy in patients with left-sided breast cancer.
