Tato bakalářská práce se zabývá ošetřovatelskou péčí o pacienta s akutním infarktem myokardu typu STEMI, který představuje nejzávažnější formu akutního koronárního syndromu. K tomuto stavu dochází v důsledku náhlého a úplného uzávěru koronární tepny, což vyvolává transmurální ischemii postihující celou tloušťku srdeční stěny. Vzhledem k rozvíjející se nekróze myokardu již po 20-30 minutách trvající ischemie se stává čas kritickým faktorem. Každá minuta bez dodávky kyslíku znamená další ztrátu funkční svaloviny. V teoretické části práce jsou zpracovány základní poznatky o anatomii a fyziologii srdce, se zvláštním zřetelem na koronární řečiště a elektrickou aktivitu myokardu. Dále je popsána patofyziologie vzniku trombu na podkladě aterosklerózy, diagnostické postupy, jako je 12svodové EKG, stanovení kardiálních biomarkerů a koronarografie. Práce rovněž mapuje akutní léčebné postupy, kde dominuje PCI, farmakoterapii a management možných komplikací, včetně arytmií, srdečního selhání či psychických dopadů onemocnění. Praktická část práce je věnována kazuistikám konkrétních pacientů, na nichž je demonstrována aplikace ošetřovatelského procesu v reálné klinické praxi. This bachelor's thesis deals with the nursing care of patient with an acute ST-segment elevation myocardial infarction (STEMI), which represents the most severe form of acute coronary syndrome. This condition occurs as a result of a sudden and complete occlusion of a coronary artery, causing transmural ischemia that affects the entire thickness of the heart wall. Given that myocardial necrosis develops after just 20-30 minutes of ongoing ischemia, time is critical factor, every minute without an oxygen supply means further loss of functional muscle tissue. The theoretical part of the thesis elaborates on the basic knowledge of the anatomy and physiology of the heart, with special attention to the coronary circulation and the electrical activity of the myocardium. Furthermore, it describes the pathophysiology of thrombus formation based on atherosclerosis, diagnostic procedures such as a 12-lead ECG, which is performed within 10 minutes of medical contact, the determination of cardiac biomarkers, and coronary angiography. The thesis also maps acute treatment procedures, dominated by primary percutaneous coronary intervention (PCI), pharmacotherapy, and the management of possible complications, including arrhythmias, heart failure, and the psychological impact of the disease.
