Tato rešeršní bakalářská práce se zabývá rizikovými faktory vzniku karcinomu štítné žlázy se zaměřením na současný stav poznání, sílu dostupných důkazů a možné etiopatogenetické mechanismy. Cílem práce bylo systematicky zpracovat a kriticky zhodnotit faktory, které mohou přispívat ke vzniku tohoto onemocnění, a současně upozornit na metodologická omezení jejich interpretace. Teoretická část práce stručně shrnuje anatomii, histologii a fyziologii štítné žlázy, základní klasifikaci jejích nádorů, diagnostické postupy a epidemiologické souvislosti včetně fenoménu nadměrné diagnostiky. Stěžejní část práce je věnována analýze jednotlivých rizikových faktorů. Za nejlépe doložený rizikový faktor je považována expozice ionizujícímu záření, zejména v dětském věku. Další významnou roli hrají genetické predispozice, rodinná anamnéza a vybrané hereditární syndromy. Diskutovány jsou rovněž autoimunitní onemocnění štítné žlázy, hormonální a reprodukční vlivy, jodový status, obezita, metabolické faktory, environmentální expozice, chemické polutanty a paradoxní vztah kouření ke karcinomu štítné žlázy. Výsledky ukazují, že etiologie karcinomu štítné žlázy je komplexní a multifaktoriální a že jednotlivé faktory se výrazně liší silou dostupných důkazů. This review-based bachelor thesis focuses on risk factors associated with the development of thyroid cancer, with particular emphasis on current knowledge, the strength of available evidence, and possible etiopathogenetic mechanisms. The aim of the thesis was to systematically summarize and critically evaluate factors that may contribute to the development of this disease, while also highlighting the methodological limitations affecting their interpretation. The theoretical part briefly outlines the anatomy, histology, and physiology of the thyroid gland, the basic classification of thyroid tumors, diagnostic approaches, and epidemiological context, including the phenomenon of overdiagnosis. The main part of the thesis is devoted to the analysis of individual risk factors. Exposure to ionizing radiation, especially during childhood, is considered the best-established risk factor. Other important determinants include genetic predisposition, family history, and selected hereditary syndromes. The thesis also discusses autoimmune thyroid disease, hormonal and reproductive influences, iodine status, obesity, metabolic factors, environmental exposures, chemical pollutants, and the paradoxical association between smoking and thyroid cancer.
