[cze][eng] Východiska: Difuzní magnetická rezonance představuje neinvazivní metodu umožňující hodnotit mikrostrukturální vlastnosti mozkové tkáně na základě pohybu molekul vody. Pokročilé difuzní modely, jako jsou DTI, DKI a NODDI, poskytují podrobnější informace o tkáňové organizaci než konvenční MRI sekvence. U pacientů s fokální strukturální epilepsií mohou být kromě vlastní epileptogenní léze ovlivněny také temporolimbické struktury, zejména hipokampus a amygdala. Cíle: Cílem práce bylo zhodnotit, zda u pediatrických pacientů s fokální strukturální epilepsií dochází k měřitelným mikrostrukturálním změnám v oblasti hipokampu a amygdaly. Porovnávány byly difuzní parametry mezi hemisférou s epileptogenní lézí a kontralaterální hemisférou bez zjevného strukturálního postižení. Metodika: Retrospektivně byl analyzován soubor 39 pediatrických pacientů vyšetřených magnetickou rezonancí. U všech pacientů byla stanovena lateralita epileptogenní léze. Pomocí softwaru FreeSurfer byla provedena segmentace hipokampu a amygdaly a následně byly z těchto oblastí zájmu extrahovány parametry odvozené z modelů DTI, DKI a NODDI. Hodnoceny byly frakční anizotropie, střední difuzivita, kurtózní frakční anizotropie, střední kurtóza, frakce volné vody, frakce intracelulárního objemu a index orientační disperze. Výsledky:...Background: Diffusion magnetic resonance imaging is a non-invasive method that enables the assessment of microstructural properties of brain tissue based on the movement of water molecules. Advanced diffusion models, such as DTI, DKI, and NODDI, provide more detailed information about tissue organization than conventional MRI sequences. In patients with focal structural epilepsy, temporolimbic structures, particularly the hippocampus and amygdala, may be affected in addition to the epileptogenic lesion itself. Objective: The aim of this thesis was to evaluate whether measurable microstructural changes occur in the hippocampus and amygdala in pediatric patients with focal structural epilepsy. Diffusion parameters were compared between the hemisphere containing the epileptogenic lesion and the contralateral hemisphere without apparent structural involvement. Methods: A cohort of 39 pediatric patients examined by magnetic resonance imaging was analyzed retrospectively. In all patients, the laterality of the epileptogenic lesion was determined. Segmentation of the hippocampus and amygdala was performed using FreeSurfer software, and parameters derived from DTI, DKI, and NODDI models were subsequently extracted from these regions of interest.
