Patelární tendinopatie představuje časté a limitující onemocnění u sportovců vystavených opakovanému skokanskému zatížení. Vyznačuje se přetrvávající bolestí a sníženou funkcí kolenního kloubu. Vzhledem k častému selhání pasivních metod léčby je v současné fyzioterapii kladen důraz na aktivní přístup formou progresivního zatěžování a cílené prevence. Cílem této práce je shrnout aktuální teoretické poznatky o etiologii, biomechanice, diagnostice a konzervativních i preventivních možnostech terapie tohoto onemocnění. Praktická část formou kazuistiky elitního atleta s tříletou historií obtíží detailně demonstruje účinnost strukturované čtyřtýdenní fyzioterapeutické intervence. Ta byla založena na cíleném izometrickém cvičení a tréninku s velkým odporem v pomalém tempu (Heavy Slow Resistance - HSR). K hodnocení efektu terapie před a po intervenci byly využity měření kloubních rozsahů metodou SFTR, přístrojová tlaková dynamometrie pro objektivizaci svalové síly, specifický dotazník VISA-P a model sledování bolesti (PMM) průběžně v rámci tréninkového cyklu. Aplikovaná intervence vedla k vysoce signifikantnímu klinickému zlepšení. Skóre dotazníku VISA-P vzrostlo z 52 na 73 bodů, což výrazně překonává minimální klinicky významný rozdíl. Průměrná vnímaná bolest během specifické sportovní zátěže klesla... Patellar tendinopathy is a common and limiting disorder in athletes exposed to repetitive jumping loads, characterized by persistent pain and reduced knee joint function. Given the frequent failure of passive treatment methods, current physiotherapy emphasizes an active approach through progressive loading and targeted prevention. The aim of this thesis is to summarize current theoretical knowledge on the etiology, biomechanics, diagnostics, and conservative as well as preventive treatment options for this condition. The practical part, designed as a case study of an elite athlete with a three-year history of symptoms, demonstrates the effectiveness of a structured four-week physiotherapeutic intervention. This intervention was based on targeted isometric exercise, and Heavy Slow Resistance (HSR) training. To evaluate the therapy's effect before and after the intervention, joint range of motion measurements using the SFTR method, instrumental pressure dynamometry for objective muscle strength assessment, the specific VISA-P questionnaire, and a pain-monitoring model (PMM) during the training cycle were utilized.
