Klára Novotná Ph.D. Název diplomové práce: Vliv výběru prvního mechanického vozíku na soběstačnost osob s tetraplegií Abstrakt diplomové práce: Diplomová práce se zabývá vlivem výběru a konfigurace prvního mechanického vozíku na rozvoj soběstačnosti u osob s tetraplegií po míšní lézi. Teoretická část definuje specifika tetraplegie, postury sedu a proces preskripce vozíku v podmínkách českého zdravotního systému. Praktická část využívá smíšený výzkumný design kombinující kvantitativní dotazníkové šetření (n = 56) s využitím modifikovaného nástroje SCIM-SR a kvalitativní analýzu dvou případových studií. Výsledky prokazují, že 53,6 % respondentů muselo během prvních pěti let užívání změnit konfiguraci svého vozíku, nejčastěji v oblasti zádové opěrky a těžiště. Statistická analýza odhalila signifikantní věkovou diskriminaci v procesu výběru pomůcky (p = 0,027), kdy pacienti nad 50 let dostávali výrazně méně možností volby než mladší respondenti. Výzkum dále potvrzuje, že aktivní mechanický vozík je klíčovým faktorem pro dosažení funkčního maxima v doménách mobility a přesunů. Abstract: This diploma thesis examines the impact of the selection and configuration of the first manual wheelchair on the development of independence in individuals with tetraplegia following a spinal cord injury. The theoretical section defines the specifics of tetraplegia, the of sitting posture, and the wheelchair prescription process within the Czech healthcare system. The practical section employs a mixed-methods research design, combining a quantitative survey (n = 56) using a modified SCIM-SR tool with a qualitative analysis of two case studies. The results demonstrate that 53.6% of respondents had to reconfigure their wheelchair within the first five years of use, most frequently regarding the backrest and the center of gravity. Statistical analysis revealed significant age-related disparity in the assistive device selection process (p = 0.027), with patients over the age of 50 being provided with significantly fewer choices than younger respondents.
