Účelem této diplomové práce bylo zhodnotit spolehlivost palpační metody měření tlaku v obturační manžetě endotracheální kanyly v reálné klinické praxi. Cílem práce bylo vyhodnotit odchylku mezi palpačním odhadem nelékařského zdravotnického pracovníka a manometrickým měřením tlaku a posoudit vliv délky odborné praxe na přesnost odhadu. Studie byla realizována na kardiochirurgické jednotce intenzivní péče se souborem 40 pacientů. U každého pacienta byly po přijetí z operačního sálu měřeny tlaky v manžetě endotracheální kanyly palpační i manometrickou metodou. Získaná data byla statisticky vyhodnocena, včetně frekvence hodnot mimo doporučené rozmezí 20-30 cm H₂O. Výsledky analýzy prokázaly, že palpační metoda je v klinické praxi nespolehlivá a vykazuje vysokou míru chybovosti. V 62,5 % případů se naměřené hodnoty nacházely mimo doporučené rozmezí, přičemž byl zaznamenán výrazný trend k nadměrnému nafouknutí manžety. Statistické testování nepotvrdilo závislost mezi délkou odborné praxe NLZP a přesností palpačního odhadu; míra chybovosti byla napříč zkušenostním spektrem personálu rovnoměrná. Z výsledků vyplývá, že subjektivní palpační odhad je rizikovým faktorem, který může vést k ischemickému poškození tracheální sliznice. The purpose of this thesis was to evaluate the reliability of the palpation method for measuring pressure in the cuff of an endotracheal tube in real clinical practice. The aim was to evaluate the deviation between the palpation estimate of a non-medical healthcare professional and the manometric pressure measurement and to assess the influence of the length of professional experience on the accuracy of the estimate. The study was carried out in a cardiac surgical intensive care unit with a group of 40 patients. After admission from the operating room, the pressures in the endotracheal cannula cuff were measured by both the palpation and manometric methods in each patient. The obtained data were statistically evaluated, including the frequency of values outside the recommended range of 20-30 cm H₂O. The results of the analysis showed that the palpation method is unreliable in clinical practice and has a high error rate. In 62.5% of cases, the measured values were outside the recommended range, and a significant trend towards excessive cuff inflation was noted.
