[cze][eng] Tato práce se zabývá významem magnetické rezonance při diagnostice a plánování léčby karcinomu prostaty. Teoretická část je zaměřena na využití multiparametrické magnetické rezonance, systému PI-RADS a roli MR při biopsii, stagingu a sledování pacientů. Praktická část porovnává konturování klinického cílového objemu prostaty při plánování radioterapie na samotném CT a při využití fúze CT/MR u souboru 50 pacientů. Výsledky ukázaly, že průměrné objemové rozdíly nebyly velké, ale mezi jednotlivými pacienty se objevila variabilita. U některých pacientů došlo po využití CT/MR fúze ke zmenšení objemu, u jiných naopak k jeho zvětšení. Větší rozdíly se ukázaly zejména v oblasti apexu a báze prostaty. Práce poukazuje na to, že magnetická rezonance může pomoci zpřesnit vyznačení cílového objemu prostaty, zejména v případech, kdy samotné CT neposkytuje dostatečně přesné anatomické zobrazení.This thesis deals with the importance of magnetic resonance imaging in the diagnosis and treatment planning of prostate cancer. The theoretical part focuses on the use of multiparametric magnetic resonance imaging, the PI-RADS system, and the role of MRI in biopsy, staging, and patient follow-up. The practical part compares the contouring of the clinical target volume of the prostate in radiotherapy planning based on CT alone and with the use of CT/MR fusion in a group of 50 patients. The results showed that the average volumetric differences between the two contouring methods were not marked; however, some variability was observed among individual patients. In some patients, the use of CT/MR fusion led to a reduction in volume, while in others it resulted in an increase. More pronounced differences were observed mainly in the apex and base of the prostate. The thesis shows that magnetic resonance imaging can contribute to more precise delineation of the prostate target volume, especially in cases where CT alone does not provide sufficiently accurate anatomical visualization.
