1 ABSTRAKT Cílem diplomové práce bylo identifikovat překážky, které podle zaměstnanců různých profesí bránily úspěšné implementaci paliativní péče v domově se zvláštním režimem. Výzkum byl realizován formou polostrukturovaných rozhovorů se šesti respondenty z odlišných profesí. Analýza ukázala, že jednotlivé profese vnímají překážky rozdílně: lékařka zdůrazňovala klinická rizika a absenci postupů, fyzioterapeutka chaotické řízení a slabou týmovou kulturu, sociální pracovnice selhání vedení, zdravotní sestra přetížení a nedostatek podpory, dietní sestra absenci systému a vzdělávání a pracovník v sociálních službách ztrátu garanta a rozpad týmu. Společným jmenovatelem byla nedostatečná koordinace, chybějící jednotná vize a slabá podpora vedení. Práce navrhuje opatření zaměřená na posílení koordinace, vzdělávání, týmové spolupráce a organizačního zázemí paliativní péče. The aim of this thesis was to identify the barriers that, according to employees from different professional groups, prevented the successful implementation of palliative care in a residential facility with a special regime. The research was conducted through semi structured interviews with six respondents representing various professions (physician, nurse, dietitian, physiotherapist, social worker, and social services worker). The analysis revealed that each profession perceived the obstacles differently: the physician emphasized clinical risks and the absence of clear procedures; the physiotherapist pointed to chaotic management and weak team culture; the social worker highlighted leadership failures and an unqualified coordinator; the nurse stressed staff overload and lack of support; the dietitian noted the absence of systematization and education; and the social services worker described the loss of a project leader and the collapse of an initially motivated team. A common denominator across all professions was insufficient coordination, a missing shared vision, and weak organizational support.
