Diplomová práce přibližuje aktuální epidemiologické trendy a evidenci o efektech genderově afirmativní medicínské léčby (GAMT) v oblasti duševního zdraví. Cílem empirické části bylo zmapovat charakteristiky a klinické trajektorie u adolescentů (věk 15-20,9 let; N = 1292), kteří v letech 2000-2025 vstoupili do péče Sexuologického ústavu Všeobecné fakultní nemocnice v Praze. Výsledky potvrzují výrazný nárůst počtu pacientů s převahou osob s dívčím matričním pohlavím (AFAB; 72,7 %). Objevily se zásadní genderové rozdíly: osoby s chlapeckým matričním pohlavím (AMAB) vstupují do péče ve vyšším věku, s nižší mírou externí podpory a v rámci cíleného testování při úvodním psychologickém vyšetření (ÚPV) vykazují signifikantně horší výsledky včetně vyššího výskytu primárních komorbidit. Navzdory tomu jsou AFAB více zatíženi reaktivními obtížemi a častěji čerpají specializovanou péči v oblasti duševního zdraví. Tato diskrepance může odrážet odlišné sociální tlaky a specifika psychosexuálního vývoje související s matričním pohlavím. U skupiny pacientů s dlouhodobou GAMT nedochází k trvalému snížení psychiatrické morbidity; po zahájení léčby byl zaznamenán přechodný nárůst zátěže a následná tendence k návratu na hladiny naměřené při ÚPV, přičemž u AMAB přetrvává výraznější rozdíl oproti výchozímu stavu.... This master's thesis explores current epidemiological trends and evidence regarding the effects of gender-affirming medical treatment (GAMT) on mental health. The empirical section aims to map the clinical trajectories and characteristics of a cohort of adolescents (age 15-20.9 years; N = 1292) who entered the care of the Institute of Sexology at the General University Hospital in Prague between 2000 and 2025. The results confirm a significant increase in patient numbers, with a predominance of individuals assigned female at birth (AFAB; 72.7%). Fundamental gender differences emerged: individuals assigned male at birth (AMAB) enter care at a later age, with lower external support, and demonstrate significantly poorer outcomes in targeted psychodiagnostic testing during the initial psychological assessment (IPA), including a higher prevalence of non-reactive comorbidities. Conversely, AFAB exhibit more frequent utilization of specialized mental health care and a higher burden of reactive difficulties. This discrepancy may reflect differing social pressures and distinct psychosexual developmental trajectories related to assigned sex.
