Cílem této práce je shrnout současné poznatky o primárních imunodeficiencích se zaměřením na jejich genetický podklad, klasifikaci a možnosti léčby, především na genovou terapii jako perspektivní kauzální přístup. Hlavní poznatky: Primární imunodeficience zahrnují více než 500 geneticky definovaných poruch, které mohou postihovat vrozenou i adaptivní imunitu. Standardní léčebné postupy zahrnují podpůrnou terapii, substituční imunoglobulinovou léčbu a transplantaci hematopoetických kmenových buněk, která může mít kurativní charakter. Genová terapie představuje moderní terapeutickou strategii založenou na přenosu nebo úpravě genetické informace v cílových buňkách pacienta. Využívá virové i nevirové vektory a může být prováděna ex vivo nebo in vivo. The aim of this thesis is to summarize current knowledge on primary immunodeficiencies with emphasis on their genetic background, classification, and therapeutic options, particularly gene therapy as a promising causal treatment approach. Main findings: Primary immunodeficiencies comprise more than 500 genetically defined disorders that may affect both innate and adaptive immunity. Standard therapeutic strategies include supportive care, immunoglobulin replacement therapy, and hematopoietic stem cell transplantation, which may have a curative potential in selected cases. Gene therapy represents a modern therapeutic strategy based on the transfer or modification of genetic material in target cells.
