Milan Martínek Ph.D. Název práce: Vliv HILT na chronickou bolest temporomandibulárního kloubu Cíle: Hlavním cílem této prospektivní studie bylo zhodnotit a porovnat klinickou efektivitu vysokovýkonné laserové terapie (HILT), manuální fyzioterapie a jejich vzájemné kombinace při léčbě pacientů s chronickou temporomandibulární dysfunkcí. Metody: Do randomizované kontrolované studie bylo zařazeno 43 probandů (29 žen, 14 mužů, průměrný věk 35,9 let) s chronickou bolestí TMK. Probandi byli náhodně rozděleni do 4 ramen: skupina 1 (HILT + fyzioterapie, n=13), skupina 2 (fyzioterapie, n=10), skupina 3 (HILT, n=10) a skupina 4 (kontrolní, n=10). Před a po intervenci byly hodnoceny primární parametry: intenzita bolesti na vizuální analogové škále (VAS), rozsah aktivního otevírání úst (ROM v mm), přítomnost spoušťových bodů (TrP) ve žvýkacích svalech a tloušťka m. masseter pomocí diagnostické sonografie. V rámci vstupního vyšetření probandi vyplnili RDC/TMD dotazník. Výsledky: V rámci meziskupinového srovnání prokázala kombinovaná terapie (skupina 1) statisticky nejvýznamnější klinický efekt (p <0,001). Došlo u ní k průměrnému poklesu bolesti o 4,0 bodu a zvětšení rozsahu otevírání úst o 8,15 mm. Monoterapie (skupina 2 a 3) prokázaly shodný pokles bolesti o 1,7 bodu...(EN) Author: Bc. Milan Martínek, Ph.D. Title: The effect of HILT on chronic temporomandibular joint pain Objectives: The primary objective of this prospective study was to evaluate and compare the clinical efficacy of high-intensity laser therapy (HILT), manual physiotherapy, and their combination in the treatment of patients with chronic temporomandibular disorders Methods: This randomized controlled trial included 43 participants (29 women, 14 men, average age 35.9 years) suffering from chronic TMJ pain. Participants were randomly assigned to four arms: group 1 (HILT + physiotherapy, n=13), group 2 (physiotherapy, n=10), group 3 (HILT, n=10), and group 4 (control, n=10). Primary outcomes were assessed at baseline and post-intervention, including: pain intensity on a Visual Analog Scale (VAS), active range of mouth opening (ROM in mm), presence of trigger points (TrPs) in the masticatory muscles, and masseter muscle thickness using diagnostic ultrasound. Patients also completed the RDC/TMD questionnaire.
