Cíl: Cílem práce bylo zhodnotit komplikace předtransfuzního vyšetření v klinické praxi Transfuzního oddělení Oblastní nemocnice v Mladé Boleslavi - Klaudiánova nemocnice, identifikovat jejich příčiny a navrhnout preventivní opatření vedoucí ke zvýšení bezpečnosti transfuzní léčby. Metody: Retrospektivní analýza zahrnovala imunohematologická data z Transfuzního oddělení Klaudiánovy nemocnice v letech 2011-2025. Hodnoceny byly výsledky předtransfuzních vyšetření, zahrnující stanovení krevní skupiny, screening a identifikaci antierytrocytových protilátek a přímý antiglobulinový test. Z důvodu změny laboratorního postupu v roce 2020 byla analýza identifikovaných antierytrocytových protilátek omezena na období 2020-2025. Součástí práce bylo také hodnocení vybraných kazuistik. Výsledky: Nejčastěji byly identifikovány antierytrocytové protilátky systému Rh, zejména anti-D (158 identifikací) a anti-E (148 identifikací). Zachyceny byly i méně časté klinicky významné protilátky, například anti-Fyᵃ a anti-Vel. Analýza kazuistik potvrdila význam individuální interpretace laboratorních výsledků a využití doplňujících imunohematologických metod při řešení nejasných nálezů. The aim of this thesis was to evaluate complications of pre-transfusion testing in the clinical practice of the Transfusion Department of Klaudiánova Hospital Mladá Boleslav, identify their causes, and propose preventive measures aimed at increasing the safety of transfusion therapy. Methods: A retrospective analysis included immunohematological data obtained at the Transfusion Department of Klaudiánova Hospital Mladá Boleslav between 2011 and 2025. Evaluated parameters included the results of pre-transfusion testing comprising blood group determination, screening and identification of anti-erythrocyte antibodies, and the direct antiglobulin test. Due to a change in laboratory procedures in 2020, the analysis of identified anti-erythrocyte antibodies was limited to the period 2020-2025. The study also included the evaluation of selected case reports. Results: The most frequently identified anti-erythrocyte antibodies belonged to the Rh system, particularly anti-D (158 identifications) and anti-E (148 identifications). Less common but clinically significant antibodies, such as anti-Fyᵃ and anti-Vel, were also detected.
