Strategie na potlačení perioperativního metastazování: protistresové, protizánětlivé a migrastatické přístupy

The vast majority of cancer patients do not die from the primary tumor itself, but from the formation of metastases and secondary tumors. It is precisely during the period before, during, and after surgical removal of primary tumor that the patient is at the highest risk of developing a new secondary cancer site. Cytotoxic therapy, stress, anesthesia, analgesics, blood transfusions, and hypothermia in the operating room or postoperative inflammation are perioperative factors that significantly reduce the patient's chances of living without recurrence. This thesis describes methods for mitigating these individual factors through simple adjustments to current treatment regimens or experimentally tested drugs designed to block stress, inflammation, and the migratory capacity of cancer cells. The current approach treats metastases only after they have formed, at which point therapy may no longer be effective. However, new perioperative strategies have the potential to prevent the transition to the malignant stage of the disease in the first place, thereby saving millions of lives.
