[cze][eng] Imunoterapie v čele s inhibitory PD-1/PD-L1 přinesla novou éru onkologie a představuje značný benefit pro pacienty s nemalobuněčným karcinomem plic (NSCLC). Tato práce klade důraz na nedostatečnost prediktivních metod v podobě imunohistochemického stanovení exprese PD-L1. Tento prediktivní biomarker je zatím jediným plošně schváleným ukazatelem indikace anti-PD-1/PD-L1 terapie u pacientů s NSCLC, avšak se potýká s řadou nedostatků, především prostorovou a časovou heterogenitou. Proto je potřeba prediktivní metody modernizovat a výzkum se zaměřuje na hledání nových prediktivních biomarkerů, které by umožnily přesnější a včasnou stratifikaci pacientů. Jejich zavádění do klinické praxe však představuje další problém z důvodu financování, technologické dostupnosti a potřebě standardizace. Tato bakalářská práce shrnuje současné poznatky a přístupy, které by v budoucnu mohly vést k zavedení nových prediktivních biomarkerů do klinické praxe. Nový komplexní a udržitelný prediktivní model může poskytnout přesnější náhled na potenciální benefit z léčby u konkrétních pacientů a tím i vyšší šanci na úspěšnou terapii.Immunotherapy, led by PD-1/PD-L1 inhibitors, has brought about a new era in oncology, and represents a significant benefit for patients with non-small cell lung cancer (NSCLC). This thesis emphasizes the inadequacy of predictive methods, particularly immunohistochemical determination of PD-L1 expression. Although this biomarker remains the only broadly approved indicator for the use of anti-PD-1/PD- L1 therapy in NSCLC patients, it is associated with several shortcomings, most notably spatial and temporal heterogeneity. Therefore, current research is focusing on the search for new predictive biomarkers that would enable more accurate and timely stratification of patients. However, their implementation into clinical practice presents additional challenges related to reimbursement, technological availability, and the need for standardization. This bachelor thesis summarizes current knowledge and approaches that may, in the future, lead to the integration of new predictive biomarkers into clinical practice. A novel, comprehensive, and sustainable predictive model could provide a more accurate assessment of the potential therapeutic benefit for individual patients, thereby increasing the likelihood of successful treatment.
