Tato bakalářská práce se zabývá problematikou uplatňování holistického přístupu v péči o geriatrické pacienty v terminálním stádiu, kteří jsou hospitalizováni na oddělení následné péče. Cílem práce bylo zmapovat, jakým způsobem sestry tento celostní přístup aplikují v praxi, jaké bariéry vnímají při jeho realizaci a jak hodnotí jeho dopad na kvalitu života umírajících seniorů. Teoretická část práce definuje specifika geriatrického pacienta se zaměřením na geriatrické syndromy, zejména stařeckou křehkost (frailty) a polymorbiditu. Dále se věnuje procesu umírání v historickém i současném kontextu, principům paliativní péče, etickým výzvám na konci života a charakteristice oddělení následné péče. Práce dále charakterizuje holistický přístup v ošetřovatelství jako péči zaměřenou na biologické, psychické, sociální a spirituální potřeby pacienta. Empirická část představuje výsledky kvalitativního výzkumného šetření. Sběr dat byl uskutečněn formou polostrukturovaných rozhovorů s osmi sestrami vybraného oddělení následné péče, které splňovaly předem stanovená kritéria. Podklad pro rozhovor tvořilo celkem 31 otázek. Získaná data byla analyzována metodou otevřeného kódování a následně rozdělena do tematických kategorií a podkategorií pro lepší přehlednost. Výsledky poukazují na významné bariéry, jako jsou personální nedostatky, časová tíseň a komunikační obtíže s rodinnými příslušníky. Práce potvrzuje, že holisticky zaměřená péče, která zahrnuje například prvky bazální stimulace, canisterapie a duchovní podpory, napomáhá zachovat lidskou důstojnost a zajišťovat klidný a důstojný závěr života geriatrického pacienta. Práce může přispět ke zvýšení povědomí o této problematice a podpořit změny směřující ke zkvalitnění ošetřovatelské péče o umírající geriatrické pacienty v praxi. This bachelor's thesis addresses the issue of applying a holistic approach to the care of geriatric patients in the terminal stage who are hospitalized in a post-acute care unit. The aim of the work was to examine how nurses apply this comprehensive approach in practice, the barriers they perceive during its implementation, and how they evaluate its impact on the quality of life of dying seniors. The theoretical part of the thesis defines the specifics of the geriatric patient with a focus on geriatric syndromes, especially frailty and multimorbidity. Furthermore, it discusses the process of dying in both historical and contemporary contexts, the principles of palliative care, ethical challenges at the end of life, and the characteristics of post-acute care units. The thesis also characterizes the holistic approach in nursing as care focused on the biological, psychological, social, and spiritual needs of the patient. The empirical part presents the results of a qualitative research study. Data collection was carried out through semi-structured interviews with eight nurses from a selected post-acute care unit who met pre-established criteria. The interview guide consisted of a total of 31 questions. The obtained data were analysed using the open coding method and subsequently divided into thematic categories and subcategories for better clarity. The results highlight significant barriers, such as staffing shortages, time pressure, and communication challenges with family members. The work confirms that holistically oriented care, which includes elements such as basal stimulation, canine therapy, and spiritual support, helps to preserve human dignity and ensures a peaceful and dignified end of life for the geriatric patient. The thesis may contribute to raising awareness of this issue and support changes leading to the improvement of nursing care for dying geriatric patients in practice.
