[cze][eng] Současný stav Juvenilní idiopatická artritida je chronické zánětlivé onemocnění, které nejčastěji postihuje děti ve věku do 16 let. Juvenilní idiopatická artritida má několik forem, které se liší průběhem i závažností. Mezi nejčastější projevy patří bolesti kloubů, ranní ztuhlost, otok a únava. Velký důraz je kladen na časnou diagnostiku a vhodně stanovenou léčbu s cílem dosažení remise. Onemocnění omezuje dětské pacienty nejen v biologické, ale i v psychologické a sociální oblasti. Z tohoto důvodu je velmi důležitá komplexní péče a multidisciplinární přístup k dětskému pacientovi. Cíl práce Cílem práce je popsat bio-psycho-sociální potřeby dětského pacienta s onemocněním juvenilní idiopatická artritida. Metodologie Tato bakalářská práce využívá kvalitativní metody a analyzuje získaná data za účelem naplnění stanoveného cíle. Výzkumné šetření bylo realizováno formou hloubkových polostrukturovaných rozhovorů s rodiči a dětskými pacienty s juvenilní idiopatickou artritidou a s dětskými sestrami pracujícími v dětské revmatologické ambulanci. Celkem bylo provedeno 13 rozhovorů, které byly doslovně přepsány do textové podoby prostřednictvím Microsoft Word. Pro detailní analýzu rozhovorů byla použita metoda otevřeného kódování technikou tužka a papír dle Hendla (2016). V jednotlivých odpovědích informantů byly identifikovány opakující se odpovědi, které byly následně barevně odlišeny dle tematických oblastí. Jednotlivé oblasti, v návaznosti na cíle a výzkumné otázky, byly rozděleny do kategorií a podkategorií, podle kterých byla vytvořena jednotlivá schémata, která byla podrobně popsána. Výsledky Výsledky analýzy odpovědí na výzkumné otázky prokazují, jaké potíže dětské pacienty s onemocněním nejvíce omezující a s jakými se nejvíce setkávají. Dále výzkum potvrzuje, že onemocnění JIA velmi ovlivňuje bio-psycho-sociální potřeby a může mít dopad na život celé rodiny. Významnou roli v péči o dětského pacienta s JIA hraje spolupráce dětské sestry s multidisciplinárním týmem a individuální přístup ke každému pacientovi. Závěr Juvenilní idiopatická artritida představuje pro dětské pacienty a jejich rodiče velmi náročnou situaci, především během diagnostiky. Práce poukazuje na význam individuálního přístupu k dětskému pacientovi a na potřebu komplexní péče, která se zaměřuje na bio-psycho-sociální potřeby. Dětská sestra v péči o dětského pacienta s JIA hraje velmi významnou roli a měla by být velkou oporou jak pro dětské pacienty, tak i jejich rodiče. Doporučení pro praxi Tato bakalářská práce může být využita jako podklad pro studenty programu Pediatrické ošetřovatelství, ale také pro pedagogy, kteří se mohou s dítětem s JIA ve své třídě setkat, což může přispět k lepšímu pochopení tohoto onemocnění. Na základě výsledků výzkumného šetření této bakalářské práce byl vytvořen informační leták určený pro rodiče, u jejichž dětí byla nově diagnostikovaná JIA. Current Status Juvenile idiopathic arthritis is a chronic inflammatory disease that most commonly affects children under the age of 16. Juvenile idiopathic arthritis has several forms that differ in their course and severity. The most common symptoms include joint pain, morning stiffness, swelling, and fatigue. Great emphasis is placed on early diagnosis and appropriately tailored treatment with the goal of achieving remission. The disease limits pediatric patients not only biologically but also psychologically and socially. For this reason, comprehensive care and a multidisciplinary approach to the pediatric patient are very important. Objective of the Study The objective of this study is to describe the bio-psycho-social needs of pediatric patients with juvenile idiopathic arthritis. Methodology This bachelor's thesis employs qualitative methods and analyzes the collected data to achieve the stated objective. The research was conducted through in-depth, semi-structured interviews with parents and pediatric patients with juvenile idiopathic arthritis, as well as with pediatric nurses working in a pediatric rheumatology outpatient clinic. A total of 13 interviews were conducted, which were transcribed verbatim into text using Microsoft Word. For a detailed analysis of the interviews, the open coding method using the pen-and-paper technique according to Hendl (2016) was employed. Recurring responses were identified in the informants' individual answers and subsequently color-coded according to thematic areas. The individual areas, in line with the objectives and research questions, were divided into categories and subcategories, based on which individual diagrams were created and described in detail. Results The results of the analysis of responses to the research questions demonstrate which difficulties most severely limit pediatric patients with the disease and which they encounter most frequently. Furthermore, the research confirms that JIA significantly affects bio-psycho-social needs and can impact the lives of the entire family. Cooperation between the pediatric nurse and the multidisciplinary team, along with an individualized approach to each patient, plays a significant role in the care of pediatric patients with JIA. Conclusion Juvenile idiopathic arthritis presents a very challenging situation for pediatric patients and their parents, especially during the diagnostic phase. This thesis highlights the importance of an individualized approach to pediatric patients and the need for comprehensive care that addresses bio-psycho-social needs. The pediatric nurse plays a very significant role in the care of pediatric patients with JIA and should serve as a major source of support for both the patients and their parents. Recommendations for Practice This bachelor's thesis can serve as a resource for students in the Pediatric Nursing program, as well as for educators who may encounter a child with JIA in their classroom, which can contribute to a better understanding of this disease. Based on the results of the research conducted for this bachelor's thesis, an informational brochure was created for parents whose children have been newly diagnosed with JIA.
