[cze][eng] Tato bakalářská práce se podrobně zabývá těhotenstvím u žen s chronickým onemocněním diabetes mellitus 1. typu (dále jen DM1) Práce je rozdělena na část teoretickou a část praktickou. V teoretické části jsou na základě rešerše odborné literatury popsána specifika těhotenství u žen s DM1. Text se zaměřuje na fyziologické změny v organismu matky, vliv diabetu na vývoj plodu a potenciální rizika, jako je diabetická fetopatie či vrozené vývojové vady. Důležitý prostor je zde věnován prekoncepční přípravě, která je pro úspěšný průběh gravidity zásadní. Závěr teoretické části se věnuje managementu ošetřovatelské péče o ženu s DM1 během porodu a šestinedělí. Pro bakalářskou práci byl stanoven cíl zanalyzovat průběh těhotenství u žen s DM1 a identifikovat rizika specifická pro toto onemocnění. Pro naplnění cílů byly formulovány čtyři výzkumné otázky. K výzkumnému šetření byl použit kvalitativní výzkum. Jako metoda sběru dat byly využity polostrukturované rozhovory, které poskytly prostor pro sdílení osobních zkušeností a emocí. Otázky byly otevřené a vytvořené podle cílů bakalářské práce. Výzkumný soubor tvořilo deset žen s diagnózou DM1, které již úspěšně porodily. Analýza těchto rozhovorů přináší cenné poznatky o tom, jak těhotné ženy s DM1 vnímají a prožívají těhotenství, jak moderní technologie ovlivňují psychickou pohodu těhotných a jakou roli v celém procesu hraje edukace a podpora ze strany multidisciplinárního týmu, jehož nepostradatelnou součástí je právě porodní asistentka. Výzkumné šetření ukázalo, že i přes vysoký podíl neplánovaných početí vyústila intenzivní péče u všech respondentek v narození zdravých dětí, přičemž většinu tvořily vaginální porody (75 %). Zásadním faktorem kompenzace i psychické stability se ukázaly moderní technologie (systém kontinuální monitorace, inzulinové pumpy), které však vyžadují vysoce informovanou pacientku, neboť technická kompetence personálu a kvalita nemocničního stravování jsou respondentkami vnímány jako nedostačující. Výsledky zdůrazňují potřebu posílení koordinační a edukační role porodní asistentky v rámci multidisciplinárního týmu. This bachelor thesis examines in detail pregnancy in women with the chronic disease diabetes mellitus type 1. The thesis is divided into a theoretical and a practical part. Based on a research of professional literature, the theoretical part describes the specifics of pregnancy in women with diabetes mellitus type 1. The text focuses on physiological changes in the mother's organism, the influence of diabetes on fetal development and potential risks such as diabetic fetopathy or congenital malformations. Significant space is devoted to preconception care, which is essential for the successful course of pregnancy. The conclusion of the theoretical part addresses the management of nursing care for women with diabetes mellitus type 1 during labor and the postpartum period. The aim of the bachelor thesis was to analyze the course of pregnancy in women with diabetes mellitus type 1 and to identify risks specific to this disease. To achieve these goals, four research questions were formulated. Qualitative research was employed for the investigation. Semi-structured interviews were used as the data collection method, providing space for sharing personal experiences and emotions. The questions were open-ended and designed according to the goals of the thesis. The research group consisted of ten women diagnosed with diabetes mellitus type 1 who had already successfully given birth. The analysis of these interviews provides valuable insights into how pregnant women with T1DM perceive and experience pregnancy, how modern technologies affect their psychological well-being, and the role of education and support from a multidisciplinary team, of which the midwife is an indispensable part. The research showed that despite a high proportion of unplanned conceptions, intensive care led to the birth of healthy children for all respondents, with the majority being vaginal births (75%). Modern technologies (system continual glucose monitoring, insulin pumps) proved to be a key factor for both metabolic compensation and psychological stability; however, they require a highly informed patient, as the technical competence of the staff and the quality of hospital catering are perceived by the respondents as insufficient. The results emphasize the need to strengthen the coordination and educational role of the midwife within the multidisciplinary team.
