[cze][eng] Souhrn Úvod: Karcinom štítné žlázy (ŠŽ) je nejčastější endokrinní malignita s rostoucí incidencí celosvětově. Současný výzkum se zaměřuje na pochopení genetických příčin jeho vzniku. Cílem práce je zjistit, jak tyto genetické změny korelují s cytologickým vyšetřením, agresivitou nádoru a s výsledky chirurgické léčby. Metody: Soubor chirurgicky léčených pacientů pro onemocnění ŠŽ v období od 1. 7. 2019 do 31. 12. 2022. U pacientů s karcinomy (121 případů) byly sledované genetické změny BRAF, RAS, TERT, RET, TP53, RET/PTC, PAX8/PPARγ a NTRK. Analýza genetických změn byla provedena pomocí alelově specifických sond, panelu Thyro-ID a metody real-time PCR. Byly sledovány klinicko-patologické znaky spojené s agresivním chováním nádoru a komplikace chirurgické léčby. Výsledky: U pacientů s Bethesda III a IV byl karcinom ŠŽ přítomen v 21,4-30,2 % případů a 79 % z nich mělo alespoň jednu z pozorovaných mutací. Rozložení karcinomů bylo následující: papilární (n = 104), folikulární (n = 4), onkocytární (n = 6), špatně diferencovaný (n = 1) a medulární (n = 6). Výskyt mutací byl: BRAF (n = 66), RAS (n = 11), TERT (n = 6), RET (n = 2), TP53 (n = 2), RET/PTC (n = 5), PAX8/PPARγ (n = 1) a NTRK (n = 5). Mutace BRAF byla spojena s vyšším výskytem multifokality, lokálních metastáz a extranodálního šíření, mutace TERT s...Introduction: Thyroid carcinoma is the most common endocrine malignancy, with a continuously increasing incidence worldwide. Current research focuses on the genetic mechanisms underlying tumor development. This study aims to evaluate the association between selected genetic alterations, cytological findings, tumor aggressiveness, and surgical outcomes. Methods: Patients surgically treated for thyroid disease between July 1, 2019, and December 31, 2022, were analyzed. In 121 patients with thyroid carcinoma, genetic alterations including BRAF, RAS, TERT, RET, TP53, RET/PTC, PAX8/PPARγ, and NTRK were assessed using allele- specific probes, the Thyro-ID panel, and real-time PCR. Clinicopathological features of aggressive tumor behavior and postoperative complications were evaluated. Results: Thyroid carcinoma was detected in 21.4-30.2% of patients classified as Bethesda III and IV, with 79% carrying at least one genetic alteration. Histologically, papillary carcinoma predominated (n = 104), followed by oncocytic (Hürthle cell; n = 6), medullary (n = 6), follicular (n = 4), and poorly differentiated carcinoma (n = 1).
