Predikce Ischemické choroby na základě postižení systémových tepen - abstrakt v českém jazyce Ischemická choroba srdeční (ICHS) zůstává jednou z hlavních příčin morbidity a mortality ve vyspělých zemích. Její prevence a diagnostika jsou tradičně založeny na stanovení rizika, které vychází z konceptu rizikových faktorů. Tento odhad však i v novějších modelech vykazuje omezenou výkonnost v některých populačních skupinách, zejména u žen, mladších pacientů a nemocných s diabetem. Vzhledem k systémovému charakteru aterosklerózy jsou proto studovány alternativní přístupy založené na biomarkerech aterosklerózy, mezi nimi i možnosti predikce ICHS pomocí detekce systémového cévního postižení. Cílem této práce bylo prostřednictvím tří komplementárních studií zhodnotit přínos vybraných strukturálních a funkčních markerů systémového arteriálního postižení pro predikci přítomnosti a fenotypu ICHS a zasadit tyto nálezy do kontextu cévní fyziologie a patofyziologie. První, retrospektivní studie hodnotila vztah kalcifikací tepen prsu (BAC) detekovaných při mamografii k přítomnosti a tíži ICHS u žen podstupujících invazivní koronarografii. Přítomnost BAC nebyla po adjustaci na věk a hlavní rizikové faktory nezávisle asociována s přítomností ICHS, byla však významně spojena s difuzním koronárním postižením, zejména s nemocí... Coronary Artery Disease Prediction Based on Systemic Arteries Infliction Ischemic heart disease (IHD) remains one of the leading causes of morbidity and mortality in developed countries. Its prevention and diagnosis are traditionally based on risk prediction derived from the concept of cardiovascular risk factors. However, even contemporary risk models demonstrate limited performance in certain population subgroups, particularly in women, younger individuals, and patients with diabetes. Given the systemic nature of atherosclerosis, alternative approaches based on atherosclerosis-related biomarkers have therefore been explored, including the prediction of IHD through detection of systemic arterial infliction. The aim of this work was to evaluate, through three complementary studies, the contribution of selected structural and functional markers of peripheral arterial disease to the prediction of the presence and phenotype of IHD and to interpret these findings within the context of vascular physiology and pathophysiology.
