Souhrn Účel: Zhodnotit vliv vitamínu D na rychlost a kvalitu hojení zlomenin u dětí. Metodika: Čtyřletá prospektivní studie zdravých dětí s diafyzárními zlomeninami kostí předloktí nebo stehenní kosti. U všech byla celkem 4x vyšetřena hladina vitaminu D: v době úrazu, jeden, tři a pět měsíců po úrazu. Všichni pacienti také podstoupili RTG vyšetření (současně s odběry krve), k hodnocení hojení zlomeniny. Děti byly na začátku studie slepě rozděleny do dvou podobně velkých skupin - jedné byl po celou dobu sledování perorálně podáván cholekalciferol, druhá byla bez suplementace a tyto skupiny jsme porovnávali. Výsledky: Celkem bylo do studie zařazeno 63 dětí - 36 suplementovaných, 27 nesuplementovaných. V době úrazu mělo 42% dětí deficit vitaminu D, 36,5% snížené hladiny, pouze 21,5% mělo normální hladinu. U suplementovaných pacientů se hladiny vitaminu D během sledování statisticky významně zvýšily, na rozdíl od skupiny bez cholekalciferolu. Také hojení zlomeniny na RTG snímku bylo v suplementované skupině statisticky významně rychlejší a lepší. Při rozdělení pacientů dle vstupních hladin vitaminu D nebo typu zlomeniny jsme pozorovali rovněž významně lepší výsledky u suplementovaných skupin. Závěr: Potvrdili jsme vysokou incidenci deficitu nebo insuficience vitamínu D u zdravých dětí. Pacienti suplementovaní... Purpose: To evaluate the effect of vitamin D on the speed and quality of pediatric fracture healing. Methods: A four-year prospective study of healthy children with diaphyseal fractures of the forearm or femur. All children had their vitamin D levels tested 4 times: at the time of injury, one, three and five months after the injury. All patients also underwent X-ray follow-ups (same time as blood tests) to evaluate fracture healing. Children were, in the beginning, blindly divided into two similarly sized groups: one group was orally administered cholecalciferol throughout the follow-up, the second group was without supplementation, and we compared those groups. Results: A total of 63 children were included in the study - 36 supplemented, 27 non-supplemented. At the time of injury, 42% of children had a deficit of vitamin D, 36,5% had insufficiency, only 21,5% had normal levels. In supplemented patients, vitamin D levels increased statistically significantly during the follow-up, in contrast to the group without cholecalciferol. Fracture healing on X-ray was also statistically significantly faster and better in the supplemented group.
