Úvod: Chirurgická léčba vestibulárního schwannomu je spojena s kompletní nebo částečnou resekcí vestibulárního nervu a obvykle vede k rozvoji jednostranné akutní periferní nebo kombinované vestibulární léze, která se u pacientů projevuje poruchami stability, závratěmi a oscilopsií. Organismus na tento stav reaguje procesem centrální kompenzace, jejíž úroveň a doba potřebná k jejímu dosažení jsou vysoce individuální. Terapeuticky je možné do těchto mechanismů zasáhnout prostřednictvím vestibulární rehabilitace. Vliv specifických forem cvičení byl již potvrzen mnoha studiemi. Hypotéza a cíle práce: Naše práce se zabývá snahou pochopit souvislost mezi rozsahem chirurgické léčby, morfologickými nálezy na zobrazovacích metodách a reziduální vestibulární funkcí, jakož i možnostmi rozšíření vestibulární rehabilitace o nové modality. Jedná se o využití prehabituace pomocí intratympanálně podávaného gentamicinu a tzv. visuovestibulární formy rehabilitace s využitím optokinetické stimulace a virtuální reality. Metodika: Vzhledem k relativně nízké frekvenci tohoto onemocnění byla studie rozdělena do několika fází. Celkem bylo zahrnuto 139 pacientů. Pacienti byli hodnoceni na základě objektivních přístrojových vyšetření, zobrazovacích metod a prostřednictvím subjektivních validizovaných dotazníků před operací,... Introduction: Surgical treatment of vestibular schwannoma is associated with complete or partial resection of the vestibular nerve and usually leads to the development of a unilateral acute peripheral or combined vestibular lesion, manifested in patients by stability disorders, dizziness and oscillopsia. The organism reacts to this condition by a process of central compensation, the level of which and the time it takes to achieve it are highly individual. Therapeutically, it is possible to intervene in these mechanisms through vestibular rehabilitation. The effect of specific forms of exercise has already been confirmed by multiple studies. Hypothesis and objectives: Our work deals with the effort to understand the connection between the extent of surgical treatment, morphological findings on imaging methods and residual vestibular function, as well as the possibilities of expanding vestibular rehabilitation to new modalities. This concerns the use of prehabituation using intratympanically administered gentamicin and the so-called visuovestibular form of rehabilitation using optokinetic stimulation and virtual reality.
