[cze][eng] Zlomeniny páteře tvoří 2-5 % všech skeletálních poranění u dětí. Zatímco poranění horní krční páteře se v v pediatrické populaci vyskytují vzácně, poranění thorakolumbální páteře jsou relativně častější. Prvním cílem naší práce bylo systematicky zhodnotit odbornou literaturu zabývající se poraněním horní krční páteře u dětí. Druhým cílem bylo analyzo- vat získané rozměry C1 obratle v jednotlivých věkových kategoriích a popsat změny, které se odehrávají během růstu. Dále jsme hodnotili rozměry nezbytné k optimálnímu zavedení implantátu a věkovou hranici, od které je možné zavést 3,5mm šrouby do massae laterales C1. Třetím cílem bylo zhodnocení rozměrů a úhlů dens axis a jejich změn během růstu a dospívání. Popsali jsme, jak se mění zakřivení dětského dentu, ve kterém věku dochází ke vzniku dorzální angulace a věkovou hranici pro zavedení dvou 3,5mm šroubů do C2 dentu. Čtvrtým cílem bylo navrhnout klasifikaci, diagnostický a terapeutický postup pro nejběžnější kompresivní zlomeniny thorakolumbální páteře u dětí. Obecný rozbor problematiky poranění horní krční páteře u dětí jsme provedli formou systematického přehledu. Další oddíl disertační práce byl rozdělen na část anatomickou a radiologickou. V anatomické části jsme provedli měření 34 C1 obratlů a 43 C2 obratlů ze sbírky Anatomického ústavu 1. LF...Spinal fractures account for 2-5 % of all skeletal injuries in children. While upper cervical spine injuries are rare in the pediatric population, thoracolumbar spine injuries are more common. The first aim of our study was to systematically review literature dealing with upper cervical spine injuries in children. The second aim was to analyze measurements obtained from anatomical specimens of C1 vertebra across different age groups and describe changes that occur during growth. We also evaluated the dimensions necessary for optimal implant placement and determined the age at which 3.5mm screws can be safely inserted into massae laterales of C1. The third aim was to assess the dimensions and angles of the dens axis and their changes throughout growth and adolescence. We described how the curvature of the pediatric dens evolves, at what age the posterior dens angulation angle appears, and the minimum age at which two 3.5mm screws can be inserted into the dens of C2. The fourth aim was to propose a classification, diagnostic approach, and therapeutic protocol for the most common compressive thoracolumbar fractures in children.
