Cíl: Cílem naší práce u skupiny pacientů s anorektální malformací (ARM) byla modifikace dosud používaného vyšetření magnetickou rezonancí (MR) tak, aby bylo schopno nahradit kontrastní rentgenové vyšetření. Cílem naší práce u pacientů s Hirschsprungovou chorobou (HD) bylo vyhledat skupinu pacientů, kteří mají pozitivní mutací v RET proto-onkogenu (RET-HD) a sledovat jejich vývoj. Materiál a metody: Prospektivní studie zahrnovala 29 pacientů s ARM. U 17 pacientů byla provedena fistulografie, u 8 pacientů distální tlakový kolostomogram. Všichni pacienti následně podstoupili MR vyšetření. U 21 pacientů bylo rektum naplněno fyziologickým roztokem přes píštěl, u 8 pacientů se stomií byl fyziologický roztok aplikován do stomie. Na obrazech T2 vážených sekvencí jsme studovali délku a průběh píštěle a tvar rekta a porovnávali je s rentgenovými snímky. U 213 pacientů s Hirschsprungovou chorobou bylo provedeno molekulárně genetické vyšetření. Pro porovnání výskytu pooperačních komplikací u pacientů s mutací v RET proto-onkogenu a u pacientů bez mutace jsme zvolili analýzu párové shody. Výsledky: U 25 pacientů s ARM byla morfologie rekta, přítomnost, průběh a délka píštěle shodná na rentgenových a MR snímcích. U 4 pacientů byla provedena pouze MR bez negativního dopadu na průběh operace či operační výsledek.... Aim: The aim of our study in the group of patients with anorectal malformations (ARM) was to evaluate the possibility of substituting contrast fistulogram (FG) and distal pressure colostogram (DPCG) by a modified pelvic magnetic resonance (MRI). Second aim was to identify Hirschsprung's disease (HD) patients with RET protooncogene mutation and follow their postoperative course. Materials and methods: The prospective study involved 29 patients with ARM who underwent a modified pelvic MRI. The length, course of fistulas and rectum, and the presence of sacral anomalies were studied on MRI images and compared with images obtained by radiologic examinations. The retrospective and prospective genetic analysis of 213 patients with HD operated on between December 1979 and December 2015 were carried out. A matched case control analysis was used for comparision of the incidence of postoperative complications in HD patients with RET mutation (RET-HD) and in HD patients without any mutation. Results: Modified MRI brought identical results as contrast studies in 25 patients when related to the fistula and rectum length and course. MRI was more accurate for the detection of sacral anomalies. MRI fistulogram was the only imaging method used in the 4 most recent patients. Genetic analysis revealed 24 germline RET mutations...
