[cze][eng] Longitudinální sledování pacientů s Friedreichovou ataxií v České republice a korelace genotypu a fenotypu Abstrakt Friedreichova ataxie (FA) je vzácné neurodegenerativní onemocnění s autosomálně recesivní dědičností, typicky způsobené expanzí tripletů GAA v genu FXN. Disertační práce shrnuje longitudinální sledování 45 pacientů s FA v České republice, cílem je zhodnotit přirozený průběh onemocnění, genotyp-fenotypové korelace a potenciál inovativních biomarkerů. Pro splnění cílů práce a ověření hypotéz byly použity neurologické škály (SARA, mFARS, INAS), funkční testy (SCAFI), kognitivní hodnocení (MoCA), pacientem reportované výstupy (ADL, EQ-5D), laboratorní parametry včetně strukturálních či digitálních biomarkerů (NFL, RNFL, Redenlab). Data byla analyzována regresními modely, survival analýzami a vizualizována pomocí spaghetti grafů a boxplotů. Výsledky harmonizují s jednotlivými literárními odkazy, unikátností práce je longitudinální pohled na Friedreichovu ataxii v celé její komplexnosti na uceleném souboru pacientů, což představuje významný přínos pro budoucí směry výzkumu v mezinárodním měřítku. Závěry potvrzují význam koordinovaného multidisciplinárního sledování a mezinárodní spolupráce pro rozvoj klinických studií i personalizované péče o pacienty s FA. This dissertation summarizes longitudinal monitoring of 45 Czech patients with FA, aiming to assess the natural history, genotype-phenotype correlations, and the potential of innovative biomarkers. To address these aims and test the hypotheses, neurological scales (SARA, mFARS, INAS), functional tests (SCAFI), cognitive assessment (MoCA), patient-reported outcomes (ADL, EQ-5D), and laboratory parameters including structural and digital biomarkers (NFL, RNFL, Redenlab) were used. Data were analyzed using regression and survival models and visualized with spaghetti plots and boxplots. The findings are consistent with previous literature; the uniqueness of this study lies in its comprehensive longitudinal evaluation of FA in a well-defined national cohort. The results underscore the importance of coordinated multidisciplinary follow-up and international collaboration for the development of clinical trials and personalized care for patients with FA.
