[cze][eng] Kongenitální hypotyreóza je ze 75-85 % porucha prenatálního vývoje štítné žlázy, z 15-25 % porucha některého stupně biosyntézy tyreoidálních hormonů s normální nebo zvětšenou štítnou žlázou. Geneticky se jedná o heterogenní skupinu onemocnění. Do diplomové práce byl zahrnut retrospektivní výběr dětských pacientů, kde převažovali pacienti z Kliniky dětí a dorostu FNKV s kongenitální hypotyreózou a strumou při narození, u kterých byla struma doložena po narození sonograficky. Spolupráce byla nabídnuta i dalším pediatrickým pracovištím v ČR. Cílem diplomové práce je molekulárně-genetická analýza 47 pacientů s podezřením na kongenitální hypotyreózu, a to pomocí bioinformatického nástroje SeqOne, který umožňuje kompletní bioinformatickou analýzu a interpretaci dat z exomového sekvenování, identifikaci genetické příčiny tohoto onemocnění, klasifikaci nalezené varianty a porovnat výsledky lidského hodnocení s automatizovanou analýzou (AI). Celkem bylo identifikováno 22 variant považovaných za kauzální - 4 patogenní varianty, 4 varianty nejasného významu (VUS) a 13 nových, dosud nepopsaných variant, které vykazovaly vysokou pravděpodobnost patogenity. Nejčastěji zastoupenými geny byly: DUOX2, TSHR, TG. Dále byly přítomny varianty v genech DUOXA2, TPO, SLC5A5, KDM6A a POU1F1. Také se podařilo nalézt a potvrdit...Congenital hypothyroidism is caused in 75-85 % of cases by a disorder of prenatal thyroid development, while in the remaining 15-25 %, it results from a defect in one of the steps of thyroid hormone biosynthesis, with a normal or enlarged thyroid gland. Genetically, it represents a heterogeneous group of diseases. This thesis includes a retrospective selection of pediatric patients from the Department of Pediatrics and Adolescent Medicine, FNKV diagnosed with congenital hypothyroidism and goiter confirmed by postnatal ultrasound. Collaboration was also offered to other pediatric centers in the Czech Republic. The aim of this thesis was a molecular-genetic analysis of 47 patients with suspected congenital hypothyroidism using the bioinformatics tool SeqOne, which allows interpretation of data from exome sequencing. The focus was on identifying the genetic cause of the disease, classifying the detected variants, and comparing the results of manual assessment with automated analysis (AI). In total, 22 variants considered causative were identified - including 4 pathogenic variants, 4 variants of uncertain significance (VUS), and 13 novel, previously undescribed variants with a high likelihood of pathogenicity.
