[cze][eng] Úvod do problematiky: Obezita představuje závažný a celosvětově narůstající zdravotní problém. Její léčba je komplexní a vyžaduje multidisciplinární přístup, jehož základem jsou behaviorální intervence zaměřené na změnu stravovacích a pohybových návyků. Klíčovým faktorem pro úspěšnost těchto intervencí je motivace pacienta, kterou mohou ovlivňovat různé psychosociální a ekonomické faktory. Tato práce se zaměřuje na dosud méně prozkoumaný aspekt - vliv způsobu financování péče na výsledky redukčního programu. Cíl: Cílem práce bylo porovnat efektivitu redukčního programu hrazeného pacientem (samoplátci) s programem externě hrazeným (EH) třetí stranou (zdravotní pojišťovnou OZP) a zhodnotit vliv typu financování na úbytek antropometrických parametrů. Metodika: Byla provedena komparativní analýza retrospektivních dat 110 účastníků (N=70 ve skupině hrazené externě (EH); N=40 ve skupině samoplátců), kteří absolvovali 90denní redukční program. Pomocí nezávislých t-testů byly srovnávány průměrné úbytky hmotnosti, BMI, procentuálního podílu tuku (TT %) a absolutní tukové hmoty (TT kg). Výsledky: Skupina samoplátců dosáhla statisticky významně vyššího úbytku hmotnosti (průměrný rozdíl 1,79 kg; 95% CI [0,15; 3,44]; p = 0,033) a BMI (průměrný rozdíl 0,55 kg/mš; 95% CI [0,02; 1,07]; p = 0,042). Its treatment is complex and requires a multidisciplinary approach, the foundation of which is behavioral interventions focused on changing dietary and physical activity habits. A key factor for the success of these interventions is patient motivation, which can be influenced by various psychosocial and economic factors. This thesis focuses on a less-explored aspect - the influence of the healthcare funding model on the outcomes of a weight loss program. Objective: The aim of this thesis was to compare the effectiveness of a weight loss program funded by patients (self-payers) with a program externally funded (EH) by a third party (a health insurance company OZP) and to evaluate the impact of the funding type on the reduction of anthropometric parameters. Methods: A comparative analysis of retrospective data from 110 participants (N=70 in the externally funded (EH) group; N=40 in the self-payer group) who completed a 90-day weight loss program was conducted. Independent t-tests were used to compare mean reductions in weight, BMI, body fat percentage (BF%), and absolute fat mass (FM kg).
