Diplomová práce se zabývá kognitivními funkcemi, včetně sociální kognice (SK), a prožíváním u pacientů se spinocerebelární ataxií typu 27B (SCA27B). Přestože byl tento subtyp SCA popsán teprve v roce 2023, dle dosavadních studií se jeví být relativně častý. V ČR se v současnosti jedná o nejčastější popsanou příčinu SCA. Kognitivními a afektivními příznaky u SCA27B se nicméně studie doposud zabývaly spíše sporadicky. Literárně přehledová část diplomové práce se zaměřuje na roli mozečku v nemotorických příznacích, přičemž vychází zejména z konceptu mozečkového kognitivně-afektivního syndromu. Dále představuje dosavadní poznatky o kognitivních funkcích (včetně SK) a prožívání u pacientů s SCA. Cílem empirické části je srovnání pacientů s SCA27B (n = 19) se zdravými dobrovolníky (n = 34) a pacienty s SCA2 (n = 9) z hlediska výkonu v doménově-specifických testech kognitivních funkcí (včetně SK) a přítomnosti depresivní symptomatiky. Pacienti s SCA27B dosahovali oproti zdravým dobrovolníkům signifikantně horších výkonů v testech fatických funkcí, paměťových funkcí a exekutivních funkcí selektivně ve schopnosti změny nastavení. V testech vizuokonstruktivních funkcí a pracovní paměti, pozornosti a psychomotorického tempa byly výsledky smíšené. Rozdíly v SK (respektive teorii mysli) nebyly signifikantní.... The thesis deals with cognitive function, including social cognition (SC), and emotional experience in patients with spinocerebellar ataxia type 27B (SCA27B). Although this SCA subtype was only described in 2023, it appears to be relatively common according to existing studies. In the Czech Republic, it is currently the most frequent identified cause of SCA. However, cognitive and affective symptoms in SCA27B have so far been addressed rather sporadically. The literature review section of the thesis focuses on the role of the cerebellum in non-motor symptoms, mainly based on the concept of the cerebellar cognitive affective syndrome. It further presents existing findings on cognitive function (including SC) and emotional experience in patients with SCAs. The aim of the empirical part is to compare SCA27B patients (n = 19) with healthy volunteers (n = 34) and SCA2 patients (n = 9) in terms of performance on domain-specific tests of cognitive function (including SC) and the presence of depressive symptomatology. Compared with healthy volunteers, SCA27B patients performed significantly worse on tests of language function, memory function, and executive function selectively in the set-shifting ability.
