[cze][eng] Bezpečnost pacienta v anesteziologické péči představuje jednu z priorit moderního zdravotnictví. Sestry poskytující anesteziologickou péči hrají významnou roli při jejím zajištění, přestože jejich specifické kompetence, vnímání rizik a podíl na řízení bezpečnosti nejsou v českém prostředí dostatečně zmapovány ani teoreticky uchopeny. Chybí hlubší poznání toho, jak sestry problematiku bezpečnosti subjektivně vnímají, jak hodnotí rizikovost prováděných činností i jaká je současná praxe v oblasti kultury bezpečí na anesteziologických pracovištích. Hlavním cílem disertační práce bylo zmapovat a popsat problematiku zajištění bezpečnosti pacienta v anesteziologické péči z pohledu sester poskytujících ošetřovatelskou péči pacientům při přípravě, průběhu a bezprostředně po ukončení anesteziologické péče. Dílčími cíli bylo zjistit subjektivní názory sester na problematiku bezpečnosti pacienta a možnost jejího ovlivnění, definovat klíčové činnosti ošetřovatelské péče v anesteziologii a stanovit jejich rizikovost, a zmapovat stávající situaci v oblasti kultury bezpečí na anesteziologických pracovištích v České republice. Byl použit smíšený výzkumný design kombinující kvalitativní a kvantitativní metody ve třech sekvenčních fázích. První fáze využívala kvalitativní přístup založený na metodě zakotvené teorie podle Strausse a Corbinové s polostrukturovanými rozhovory se 14 anesteziologickými sestrami. Druhá fáze kombinovala Delphi studii se 20 experty ve dvou kolech pro definování činností anesteziologických sester a následné dotazníkové šetření pro hodnocení jejich rizikovosti a možností aplikace strategií kultury bezpečí. Třetí fáze představovala kvantitativní výzkum prostřednictvím online dotazníkového šetření u 68 nelékařských vedoucích pracovníků anesteziologických pracovišť napříč Českou republikou. Kvalitativní fáze vedla k vytvoření teorie ?Podmínky anesteziologických sester pro bezpečnou anesteziologickou péči", která identifikuje pět vzájemně propojených dimenzí: připravenou sestru, připravené pracoviště, připraveného/zajištěného pacienta, spolupráci v anesteziologickém týmu a kulturu bezpečí v organizaci. Tyto dimenze představují nezbytné předpoklady pro poskytování bezpečné anesteziologické péče. Delphi studie dosáhla konsenzu u 48 činností vykonávaných sestrami v anesteziologické péči, které byly kategorizovány do sedmi oblastí péče. Hodnocení rizikovosti odhalilo, že 69 % činností je zatíženo vysokým rizikem a 31 % středním rizikem. Mezi nejrizikovější činnosti patří asistence při zajištění průchodnosti dýchacích cest, intravenózní aplikace léčiv a příprava speciálních pomůcek. Kvantitativní šetření odhalilo významné rozdíly v implementaci strategií kultury bezpečí na anesteziologických pracovištích. Zatímco funkční systém hlášení nežádoucích událostí je dobře implementován, simulační vzdělávání představuje významnou mezeru. Statistická analýza potvrdila významné rozdíly v názorech na kulturu bezpečí podle manažerské pozice a typu zřizovatele pracoviště. Standardizované postupy jsou nejčastěji implementovanou strategií, avšak kontrolní seznamy a systémy hlášení pro specifické činnosti vykazují nižší míru implementace. Studie představuje první komplexní zmapování problematiky bezpečnosti pacienta v anesteziologické péči z pohledu sester v českém prostředí. Vytvořená teorie obohacuje teoretické poznání v oblasti ošetřovatelství a poskytuje rámec pro další výzkum i praktické aplikace. Konsenzuální definice činností anesteziologických sester může sloužit jako podklad pro legislativní úpravy a standardizaci kompetencí v oboru. Výsledky poskytují empirický základ pro prioritizaci bezpečnostních opatření a konkrétní doporučení pro zlepšení kultury bezpečí, včetně systematického zavádění simulačního vzdělávání a posílení komunikace a zpětné vazby.Patient safety in anaesthesia care represents one of the priorities of modern healthcare. Anaesthetic nurses play a significant role in ensuring patient safety, although their specific competencies, risk perception, and contribution to safety management are not sufficiently mapped or theoretically understood in the Czech context. There is a lack of deeper understanding of how anaesthetic nurses subjectively perceive safety issues, how they assess the risk of performed activities, and what the current practice is in safety culture at anaesthesia departments. The main objective of this dissertation thesis was to map and describe the issue of ensuring patient safety in anaesthesia care from the perspective of nurses providing nursing care to patients during preparation, the course of anaesthesia, and immediately after completion of anaesthesia care. The specific objectives were to determine the subjective opinions of anaesthetic nurses on patient safety issues and the possibility of influencing them, to define key nursing activities in anaesthesiology and determine their risk level, and to map the current situation in safety culture at anaesthesia departments in the Czech Republic. A mixed methods research design was used, combining qualitative and quantitative methods in three sequential phases. The first phase utilized a qualitative approach based on grounded theory methodology according to Strauss and Corbin with semi-structured interviews with 14 anaesthetic nurses. The second phase combined a Delphi study with 20 experts in two rounds to define anaesthetic nursing activities and subsequent questionnaire survey to assess their risk level and possibilities for implementing safety culture strategies. The third phase represented quantitative research through an online questionnaire survey among 68 leading staff members of anaesthesia departments across the Czech Republic. The qualitative phase led to the creation of the theory " Anaesthetic nurses' conditions for ensuring patient safety during anaesthesia care", which identifies five interconnected dimensions: prepared nurse, prepared workplace, prepared/ safeguarded patient, collaboration in the anaesthesia team, and safety culture in the organization. These dimensions represent essential prerequisites for providing safe anaesthesia care. The Delphi study achieved consensus on 48 activities performed by anaesthetic nurses, which were categorized into seven areas of care. Risk assessment revealed that 69% of activities carry high risk and 31% medium risk. The highest-risk activities include assistance in ensuring airway patency, intravenous drug administration, and preparation of special equipment. The quantitative survey revealed significant differences in the implementation of safety culture strategies at anaesthesia departments. While a functional adverse event reporting system is well implemented, simulation education represents a significant gap. Statistical analysis confirmed significant differences in opinions on safety culture according to management position and type of healthcare facility founder. Standardized procedures are the most frequently implemented strategy, but checklists and reporting systems for specific activities show lower implementation rates. The study represents the first comprehensive mapping of patient safety issues in anaesthesia care from the perspective of nurses in the Czech context. The developed theory enriches theoretical knowledge in the field of nursing and provides a framework for further research and practical applications. The consensual definition of anaesthetic nursing activities may serve as a foundation for legislative adjustments and standardization of competencies in the field. The results provide an empirical basis for prioritizing safety measures and specific recommendations for improving safety culture, including systematic implementation of simulation education, strengthening communication and feedback.
