[cze][eng] β-buňky pankreatu hrají rozhodující roli v regulaci sekrece inzulínu. Vrozené defekty v genech kódujích proteiny β-buněk potřebné pro jejich správnou funkci vedou ke vzniku vzácných forem poruch inzulínové sekrece ve smyslu zvýšené i snížené inzulínové produkce - vrozeného hyperinzulinismu a monogenních forem diabetu. Cílem dizertační práce bylo podat komplexní pohled na vliv změn ve vybraných genech na patofyziologii β-buňky a změny sekrece inzulínu s využitím funkčních studií in- vitro. Výsledky dizertační práce shrnuje celkem pět publikací o vrozeném hyperinzulinismu a novorozeneckém diabetu, konkrétně se jedná o jedno review, tři původní práce a jednu kazuistiku. Byla provedena komplexní fenotypická a genetická analýza pacientů s diagnózou vrozeného hyperinzulinismu z celé České republiky, u kterých oproti jiným populacím převažuje autozomálně dominantní typ dědičnosti. Po mutacích v genu ABCC8 jsou v našich podmínkách druhou nejčastější příčinou vrozeného hyperinzulinismu mutace v genu HNF1A. Díky funkčním studiím nových mutací v tomto genu jsme potvrdili podíl HNF1A na patofyziologii vrozeného hyperinzulinismu. Dále jsme funkčně charakterizovali celkem pět nových mutací v genu ABCC8 a prokázali jejich kauzální vliv na rozvoj hyperinzulinismu. Congenital defects in the genes encoding proteins that are necessary for the proper β-cell function are responsible for rare conditions affecting the insulin secretion: both in the sense of increased and decreased insulin production - congenital hyperinsulinism and monogenic forms of diabetes, respectively. The aim of this thesis was to give a comprehensive overview of the influence of changes in the selected genes on the β-cell pathophysiology and its effect on insulin secretion using in-vitro functional studies. The results of the PhD work are summarized in five publications focusing on congenital hyperinsulinism and neonatal diabetes, specifically one review, three original papers and one case report. We carried out a comprehensive phenotypic and genetic analysis of patients with the diagnosis of congenital hyperinsulinism from the whole Czech Republic. In comparison with other populations, the autosomal dominant mode of inheritance is more prevalent in our country. Following mutations in the ABCC8 gene, the second most common cause of congenital hyperinsulinism in the Czech Republic are autosomal dominant mutations in the gene HNF1A.
