The role of "point-of-care" methods in the diagnostics of primary hemostasis disorder in ECMO patients Abstract This dissertation addresses the development and dynamics of primary hemostasis impairment associated with intraoperative short-term veno-arterial extracorporeal membrane oxygenation (VA ECMO) support during bilateral lung transplantation. The aim of this study was to determine whether point-of-care diagnostic methods, including impedance aggregometry (Multiplate® , ROTEM® Platelet) and platelet function analyzer (INNOVANCE PFA-200® ), could identify early hemostatic dysfunction during ECMO support, and to characterize the timing of dysfunction onset and resolution. Laboratory assays for von Willebrand factor (vWF) quantity, functional activity, and multimeric structure were performed in parallel with functional testing. The results demonstrated a rapid and significant impairment of platelet function, detectable shortly after ECMO initiation across all bedside testing platforms, with partial recovery after ECMO discontinuation. Quantitative and functional analyses of vWF showed elevated post-ECMO levels without evidence of clinically relevant acquired von Willebrand disease. Význam "point-of-care" diagnostiky poruchy primární hemostázy u pacientů na ECMO Abstrakt Disertační práce se zabývá rozvojem a dynamikou poruch primární hemostázy spojených s krátkodobou podporou pomocí veno-arteriální mimotělní membránové oxygenace (VA-ECMO) v průběhu operačního výkonu při provedení oboustranné plicní transplantace. Cílem práce bylo zjistit, zda lze poruchu primární hemostázy včas detekovat pomocí vyšetřovacích metod dostupných u lůžka pacienta, které zahrnují impedanční agregometrii (Multiplate® , ROTEM® Platelet) a analyzátor funkce destiček (INNOVANCE PFA-200® ), a charakterizovat časový průběh vzniku a ústupu této poruchy. Současně byla provedena laboratorní analýza množství, funkce a multimerní struktury von Willebrandova faktoru (vWF). Výsledky prokázaly rychlý a významný pokles funkce destiček zjistitelný krátce po zahájení ECMO podpory ve všech používaných diagnostických metodách s částečnou obnovou po ukončení podpory. Analýza hladiny a funkce vWF objevila zvýšené hladiny po ukončení ECMO podpory, bez průkazu klinicky významné získané von Willebrandovy choroby. Tato zjištění poskytují podrobnou charakteristiku dynamických změn primární hemostázy během krátkodobé ECMO podpory a podporují využití těchto metod v monitorování funkce destiček v průběhu chirurgického výkonu...
