[cze][eng] Bakalářská práce se zabývá ošetřovatelskou péčí o předčasně narozené novorozence, tedy novorozence, které se narodili před ukončeným 37. gestačním týdnem. Tito novorozenci jsou ošetřováni na jednotkách intermediární péče nebo na jednotkách intenzivní péče. Ošetřovatelské intervence jsou zaměřeny na zajištění potřeb novorozenců, kam patří potřeba zajištění tepla, dýchání, spánku, výživy, vylučování, cévního vstupu, lásky, bezpečí a potřeba být bez bolesti. Práce se zabývá uspokojováním těchto potřeb a zapojením matek v péči o ně. Cílem bylo zjistit, jak se zapojují matky do péče o nedonošeného novorozence z pohledu sester a z pohledu matek. Výzkum probíhal kvalitativně, metodou dotazování, technikou hloubkového rozhovoru. Výzkumným souborem byly matky nedonošených novorozenců a sestry pracující na jednotce intermediární péče. Jednalo se o záměrný výběr. Jejich odpovědi byly zapisovány a následně analyzovány metodou "tužka a papír" (Švaříček & Šeďová, 2014). Výsledky byly rozděleny na kategorie výsledků získaných od matek a od sester. Z výsledků od matek vznikla první kategorie zaměřující se na metody edukace, návrhy na zlepšení edukace, edukaci ke správné manipulaci a k čerpání informací z jiných zdrojů. Druhá kategorie se týká hospitalizace novorozence bez matky. Třetí kategorie se zaměřuje na zapojení blízkých a na péči o potřeby novorozence z pohledu matek. Čtvrtá kategorie je zaměřená na potřeby matek, které jsou dlouho hospitalizované v nemocnici společně s nedonošeným novorozencem. Do této kategorie je zahrnuta i komunikace se sestrami, zázemí pro matky, podpora laktace, poskytnutí informací o návazné zdravotní péči. Poslední kategorie je cílená na vzájemnou podporu mezi matkami. Z výsledků od sester se první kategorie zaměřuje na edukaci v péči o novorozence z pohledu sester. Druhá kategorie se týká úlohy sester při zapojování matek do péče. Třetí kategorie je zaměřena na uzpůsobení péče dle stavu matky. Zaměřuje se na potřeby matek, na odreagování matek od nemocničního prostředí a na překážky v péči ze stran matek a ze stran novorozence. Výsledky ukazují, že odpovědi matek a sester se v mnoha ohledech shodují. Matky i sestry popisovaly podobné možnosti zapojení, zejména v oblasti ošetřovatelské péče. Matky se zapojují a jsou zároveň sestrami zapojovány v kojení, dokrmování, vážení, koupání a přebalování novorozence. Některé matky se zapojují do fyzioterapie s novorozencem a všechny matky se zapojují do klokánkování. Edukace matek je ze stran sester i matek vnímána jako dostatečná. Informantky zmiňovaly, že by se edukace mohla vylepšit například formou videí či brožurek, ale není to nutností. Z výsledků vyplývá, že překážkou v zapojení bývá úzkost matek či zdravotní stav novorozence. U matek je podporována psychická stabilita komunitním setkáním, propustkami a psychologem. Pro matky by byla péče příjemnější, pokud by měly v nemocničním prostředí více soukromí. Práce může být zdrojem informací jak pro sestry, tak i pro matky. Přináší podněty k zamyšlení, jak dále rozvíjet zapojení matek do péče na jednotkách intermediární péče. Pro matky to může být zdrojem inspirace a návodem, jak se aktivně zapojit do nemocniční péče o nedonošeného novorozence.This bachelor thesis focuses on the nursing care of premature newborns, meaning newborns born before the 37th gestational week. These newborns are taken care of in intermediate care units or intensive care units. Nursing interventions are focused on providing the needs of newborns, which include the need to provide warmth, breathing, sleep, nutrition, excretion, vascular access, love, security, and the need of being pain-free. This thesis explores the satisfaction of these needs and the involvement of mothers in their care. The aim of the study was to find out how mothers are involved in the care of the premature newborn from the nurses' perspective and from the mothers' perspective. The research was conducted qualitatively, using the questioning method, in-depth interview techniqu. The research sample consisted of mothers of premature newborns and nurses working in the intermediate care unit, selected through purposive sampling. The responses were recorded in data collection sheets and subsequently analyzed using the "pen and paper" method (Švaříček & Šeďová, 2014). The results were divided into categories based on data collected from mothers and from nurses. The results from mothers formed the first category, which focuses on methods of education, suggestions for improving education, education of proper handling of the newborn, and sourcing information from other sources. The second category relates to the hospitalization of a newborn without the mother. The third category focuses on the involvement of close family members and on tending to the needs of the newborn from the mothers' perspective. The fourth category focuses on the needs of mothers who are hospitalized for an extended period together with their premature newborn. This category also includes communication with nurses, facilities for mothers, lactation support. The final category targets mutual support among mothers. The fourth category focuses on the needs of mothers who are hospitalized with their premature baby for extended periods. The final category dealt with mutual support among mothers. This category includes communication with nurses, maternal facilities, lactation support, and provision of information regarding follow-up healthcare. The final category deals with mutual support among mothers. The results from nurses formed a first category focused on education in newborn care from the nurses' perspective. The second category concerns the role of nurses in involving mothers in the care process. The third category explores the adaptation of care to the condition of the mother, addressing their needs, helping them cope with the hospital environment and identifying barriers to care from the mothers' and the newborns' sides. The results show that mothers' and nurses' responses are consistent in many respects. Both groups described similar opportunities for involvement, particularly in the areas of nursing care and education. Mothers participate and are actively involved by nurses in breastfeeding, supplementary feeding, weighing, bathing, and in kangarooing. Education of mothers is perceived as sufficient by nurses as well as mothers. Informants mentioned that education could be improved, for example, through videos or brochures, but it is not considered essential. The results show that maternal anxiety or the newborn's health condition are often barriers to the involvement. Mothers' psychological stability is supported through community meetings, temporary leaves from the hospital, and psychological support. This thesis can serve as a valuable resource for both nurses and mothers, offering insights into how maternal involvement in intermediate care units can be further developed. For mothers it can be a source of inspiration and practical guidance on how to take an active role in the hospital care of their premature newborn.
