[cze][eng] Adenotomie je chirurgický zákrok, při kterém dochází k odstranění nosních mandlí. Nejčastěji se provádí u dětí, které trpí opakovanými infekcemi horních cest dýchacích, poruchami spánku nebo dýchacími obtížemi. Tento výkon je běžnou součástí dětské otorhinolaryngologie a jeho úspěšnost do značné míry závisí na následné pooperační péči. Ošetřovatelská péče hraje klíčovou roli nejen v nemocniční fázi rekonvalescence, ale i po propuštění dítěte do domácího prostředí. Zajištění dostatečné informovanosti rodičů o zásadách domácí péče je proto nezbytné pro prevenci možných komplikací, jako jsou pooperační krvácení, bolest nebo infekce. Hlavním cílem této bakalářské práce je podrobně zmapovat problematiku ošetřovatelské péče o děti po adenotomii a identifikovat edukační potřeby rodičů v této oblasti. Konkrétně se práce zaměřuje na následující cíle: Prvním cílem je zmapovat specifika ošetřovatelské péče u dětí po adenotomii, a to jak v nemocničním prostředí, tak v domácí péči. Druhým cílem je zmapovat edukační potřeby rodičů týkající se pooperační péče o dítě, včetně prevence možných komplikací. K dosažení cílů byl zvolen kvalitativní výzkum, který poskytl hlubší pohled na zkušenosti rodičů a ošetřovatelského personálu s pooperační péčí o děti po adenotomii. Data byla získána pomocí hloubkových polostrukturovaných rozhovorů se dvěma skupinami informantů - sestrami z dětského pooperačního oddělení a rodiči dětí po adenotomii. Celkem bylo uskutečněno 12 rozhovorů (7 se sestrami, 5 s rodiči), které byly nahrány, přepsány a analyzovány metodou "tužka a papír". Pomocí kódování byly vymezeny tematické celky, jež sloužily k identifikaci edukačních potřeb rodičů a zhodnocení postupů pooperační péče. Výzkum probíhal od konce února do konce března 2025. Z kvalitativního výzkumu zaměřeného na ošetřovatelskou péči u dětí po adenotomii vyplynulo, že většina rodičů byla s péčí spokojena, zejména s přístupem sester během hospitalizace. Rodiče oceňovali otevřenou komunikaci, vstřícnost a empatii zdravotníků, které jim pomáhaly lépe zvládat obavy spojené s operací dítěte. Zároveň se ukázalo, že některé informace byly podány příliš stručně - rodičům chyběly podrobnější instrukce ohledně možných komplikací, domácí péče, stravy a zvládání bolesti. I když obdrželi základní doporučení, často si v nich nebyli jistí. Za velmi přínosné by považovali možnost kontaktovat konkrétního zdravotníka i po propuštění domů, například sestru či lékaře, který by jim byl k dispozici během rekonvalescence dítěte. Tato práce je určena především rodičům dětí podstupujících adenotomii a poskytuje jim přehledné a srozumitelné informace o průběhu zákroku, pooperační péči a možných komplikacích. Zároveň slouží jako podpora pro zdravotnický personál - zejména dětské sestry - při edukaci rodičů v předoperačním i pooperačním období. Práce je rovněž cenným zdrojem pro studenty zdravotnických oborů, kteří se chtějí seznámit s tématem dětské adenotomie z pohledu ošetřovatelské péče a edukace.Adenotomy is a surgical procedure in which the tonsils are removed. It is most often performed in children who suffer from recurrent upper respiratory tract infections, sleep disorders or breathing difficulties. This procedure is a common part of paediatric otorhinolaryngology and its success depends largely on the follow-up care. Nursing care plays a key role not only in the hospital recovery phase but also after the child is discharged to the home environment. Ensuring that parents are sufficiently informed about the principles of home care is therefore essential to prevent potential complications such as postoperative bleeding, pain or infection. The main aim of this bachelor thesis is to map in detail the issue of nursing care for children after adenotomy and to identify the educational needs of parents in this area. The first aim is to map the specifics of nursing care for children after adenotomy, both in the hospital setting and in home care. The second aim is to map the educational needs of parents regarding postoperative care of their child, including prevention of possible complications. To achieve the objectives, qualitative research was chosen to provide a deeper insight into the experiences of parents and nursing staff with post-operative care of children after adenotomy. Data were collected through in-depth semi-structured interviews with two groups of informants - nurses from the paediatric post-operative ward and parents of children after adenotomy. A total of 12 interviews were conducted (7 with nurses, 5 with parents), which were recorded, transcribed and analysed using the pencil and paper method. Thematic units were identified using coding to identify the educational needs of parents and to assess postoperative care practices. The research was conducted from the end of February to the end of March 2025. Qualitative research on nursing care for children after adenotomy showed that most parents were satisfied with the care, especially with the nurses' approach during hospitalization. Parents appreciated the open communication, helpfulness and empathy of the nurses, which helped them to better manage their child's fears related to the surgery. At the same time, some information appeared to be given too briefly - parents lacked more detailed instructions about possible complications, home care, diet and pain management. Although they received basic recommendations, they were often unsure about them. They would have found it very helpful to be able to contact a specific health professional after discharge home, such as a nurse or doctor, who would be available to them during the child's recovery. This work is intended primarily for parents of children undergoing adenotomy and provides them with clear and understandable information about the procedure, postoperative care and possible complications. At the same time, it serves as a support for medical staff - especially paediatric nurses - in educating parents in the pre- and post-operative period. The work is also a valuable resource for medical students who wish to learn about the topic of paediatric adenotomy from the perspective of nursing care and education.
