Význam molekulárně genetických metod v diagnostice neuroinfekcí Úvod: Neuroinfekce představují závažná onemocnění s možností rozvoje trvalých následků případně i úmrtím. Cíl práce: Stanovení přínosu molekulárně genetických metod v diagnostice neuroinfekcí a stanovení zastoupení potenciálně preventabilních původců Metodika: Prospektivní studie zahrnovala 1463 pacientů, kteří byli hospitalizovaní v letech 2004 - 2013 na Klinice infekčního lékařství v Ostravě, z toho 283 pacientů s purulentní meningitidou a 1180 pacientů s aseptickou meningitidou. Kritériem pro stanovení diagnózy byl počet elementů v mozkomíšním moku převyšující 5 elementů/1 µl. V diagnostice purulentních meningitid byla použita kultivace likvoru i hemokultury, polymerázová řetězová reakce (PCR) k detekci Streptococcus pneumoniae, Haemophilus influenzae typ b, Neisseria meningitidis a Listeria monocytogenes. V diagnostice aseptických meningitid byly použity sérologické metody k průkazu infekce virem klíšťové encefalitidy, enterovirů, epidemické parotitidy, Mycoplasma pneumoniae a lymeské borreliózy. PCR bylo využito k vyšetření enterovirů, epidemické parotitidy, viru hepatitidy E (VHE), chřipky a herpetických virů. Výsledky: V souboru 283 pacientů s purulentní meningitidou byla etiologie objasněna u 73 % pacientů, PCR vyšetření bylo... The role of molecular genetics methods in diagnostics of neuroinfections Background: Neuroinfections are serious diseases. They are associated with frequent development of permanent, debilitating consequences or even death. Aim: The aim of this work was to determine possible contribution of molecular genetic methods to diagnosis of neuroinfections and define potentially preventable agents. Methods: The prospective study included 1,463 patients who were hospitalized at the Department of Infectious Diseases in Ostrava in the years 2004-2013 of which 283 patients with purulent meningitis and 1,180 patients with aseptic meningitis. The criterion for the diagnosis was the number of elements in cerebrospinal fluid which had to exceed 5x106 /l. The diagnosis of purulent meningitis was established by cultivation, PCR for the detection of Streptococcus pneumoniae, Haemophilus influenzae, Neisseria meningitidis, and Listeria monocytogenes. The diagnosis of aseptic meningitis was established by serological tests for tick-borne encephalitis, enterovirus, mumps, Mycoplasma pneumoniae, and Lyme disease. PCR was used to investigate enterovirus, influenza virus, VHE and herpes viruses.
