Transplantace orgánů představuje pro řadu pacientů terapeutickou strategii pro konečné stádium poškození orgánů a stejně tak je tomu i v případě transplantace ledvin. Dlouhodobého přežití transplantovaného orgánu však nelze dosáhnout bez použití imunosupresivních léků, látek, které jsou nutné k utlumení imunitního systému a prevenci rejekce. Tyto léky mají však řadu vedlejších účinků a absence personalizovaných schémat jejich používání představuje stále velký problém. V důsledku toho přetrvává potřeba imunologických biomarkerů, které by byly schopny rychle, přesně, levně a neinvazivně identifikovat pacienty s počínajícím poškozením aloštěpu a posoudily aktuální imunitní stav příjemce, což by umožnilo i lepší individualizovanou imunosupresi. Přínosné se nám v tomto ohledu zdá hodnocení sérových hladiny vybraných prozánětlivých cytokinů rodiny IL-1 a chemokinového profilu u pacientů po transplantaci ledvin. Naše studie prokázala potransplantační změny v sérových koncentracích IL-18, jehož hodnoty jsou zvýšené právě u pacientů s rejekcí, a naopak koncentrace jeho antagonisty, IL- 18BP, v čase rejekce klesají. Slibné by mohlo být i hodnocení chemokinů CXCL1, CXCL9, CXCL10, CXCL11 a CX3CL1, jejichž sérové koncentrace jsou vyšší u pacientů s rejekcí. Zdroj jednotlivých chemokinů a schopnost... Organ transplantation represents a therapeutic strategy for many patients in the final stages of organ failure and the same applies to kidney transplantation. However, long-term survival of the transplanted organ cannot be achieved without the use of immunosuppressive drugs, which are necessary to suppress the immune system and prevent rejection. These drugs, however, have many side effects, and the lack of personalized dosing regimens remains a significant problem. As a result, there is a persistent need for immunological biomarkers that can quickly, accurately, cheaply, and non-invasively identify patients with early graft damage and assess the recipient's current immune status, allowing for better individualized immunosuppression. In this regard, the evaluation of serum levels of selected pro-inflammatory cytokines from the IL-1 family and the chemokine profile in patients after kidney transplantation appears beneficial. Our study demonstrated post-transplant changes in serum concentrations of IL-18, which are elevated in patients experiencing rejection, while the concentration of its antagonist, IL-18BP, decreases during rejection. The evaluation of chemokines CXCL1, CXCL9, CXCL10, CXCL11, and CX3CL1, whose serum concentrations are higher in patients with rejection, also appears promising. The...
