Fibrilace síní (FS) je vůbec nejčastěji léčenou poruchou srdečního rytmu. Úspěšnost radiofrekvenční katétrové ablace se při léčbě paroxysmální FS pohybuje mezi 60 % a 80 %. Cílem této práce je určit parametry spojené s recidivou FS, díky kterým by bylo možné identifikovat pacienty se zvýšeným rizikem na opakovaný výskyt této arytmie. Zpracována byla data pacientů s paroxysmální FS, kteří podstoupili radiofrekvenční ablaci prostou izolací plicních žil. Z celkového počtu 98 pacientů byla diagnostikována recidiva FS u 19 z nich. Recidivující a nerecidivující pacienti se statisticky významně lišili v hodnotách celé řady zátěžových a echokardiografických parametrů, které byly dále využity v regresní analýze. Zvláště zkoumaný byl vliv vrcholové spotřeby kyslíku (pVO2) na úspěšnost ablace. Tento parametr se ukázal jako silný prediktor recidivy fibrilace síní po adjustování pro možné zavádějící faktory pohlaví a věku (poměr rizik 0,43). Jako ideální kombinace příznaků ve vícenásobném regresním modelu se ukázala čtveřice parametrů: pVO2, vrcholová rychlost pohybu mitrálního annulu v pozdní diastole levé komory měřená septálně, systolický krevní tlak po zátěži a index objemu levé síně. Výsledky této práce potvrzují přínos zejména zátěžových a echokardiografických parametrů, které mohou nést důležitou prognostickou informaci v souvislosti s úspěšností ablačního zákroku.
Atrial fibrillation (AF) is the most frequently treated heart arrhythmia. Radiofrequency catheter ablation is a treatment option with a success rate ranging from 60 % to 80 % for paroxysmal AF. This work aimed to determine parameters associated with AF recurrence to identify high-risk patients. Data from 98 patients who underwent pulmonary vein isolation were analyzed. Out of these patients, 19 experienced AF recurrence. Exercise and echocardiographic parameters differed significantly between the recurrence and non-recurrence groups and were used in regression analysis. Peak oxygen consumption (pVO2) was found to be a strong predictor of AF recurrence after adjusting for gender and age (hazard ratio 0.43). Four parameters were identified as the ideal combination in multivariable analysis: pVO2, septal peak late diastolic mitral annulus velocity, post-exercise systolic blood pressure, and left atrial volume index. These findings highlight the importance of stress and echocardiographic parameters in predicting the success of ablation procedures.
