Souhrn Úvod: Diabetická retinopatie (DR) a diabetický makulární edém (DME) jsou závažné komplikace diabetes mellitus (DM). DME je nejčastějším projevem DR, který způsobuje pokles vízu. Tato práce prezentuje výsledky sledování pacientů s DME po léčbě mikropulsním laserem s vlnovou délkou 577nm. Metody a soubor: Do souboru bylo zařazeno celkem 167 očí 111 pacientů s DME, všichni pacienti byli sledováni minimálně 1 rok. Celkem 70 očí 47 pacientů bylo sledováno 3 roky a 52 očí 34 pacientů bylo sledováno 5 let. Všichni pacienti v souboru nesplňovali kritéria pro úhradu léčby DME intravitreálním podáním anti-vaskulárního endoteliálního růstového faktoru (anti-VEGF) nebo si ji nepřáli podstoupit. Před zařazením do studie bylo u všech očí provedeno ošetření sítnice mimo makulu standardním laserem podle typu diabetické retinopatie (DR). Při vstupu do studie bylo provedeno ošetření DME mikropulsním laserem. Kontroly následovaly každé 3 měsíce v prvním roce a každých 4 až 6 měsíců v následujících letech. Ošetření laserem bylo podle potřeby opakováno, léčba nebyla kombinována s fokálním laserem makuly ani anti- VEGF terapií. Výsledky: V souboru sledovaném 1 rok byla centrální tloušťka sítnice (CRT) na počátku průměrně 378,8 ± 133,0 µm a po roce 333,9 ± 126,8 µm (rozdíl -45,0 ± 143,3 µm), objem makuly (TMV) byl na... Micropulse laser therapy of cystoid macular edema Introduction: Diabetic retinopathy (DR) is a typical microvascular complication of diabetes mellitus (DM). The most common manifestation of DR that reduces vision is diabetic macular edema (DME). The objective of this study was to evaluate the efficacy of DME therapy using subthreshold micropulse laser (SMPL) with a wavelength of 577 nm. Cohort and methods: The cohort with a monitoring period of 1 year included the total number of 167 eyes of 111 patients with DME. The cohort with a monitoring period of 3 years included the total number of 70 eyes of 47 patients. The cohort with a monitoring period of 5 years included the total number of 52 eyes of 34 patients. None of the patients met the reimbursement criteria for DME treatment with intravitreal administration of anti-VEGF or did not wish to undergo that treatment. All the eyes underwent treatment of the retina outside the macula with a standard laser pulse therapy. The treatment depended on the type of DR.
