Kognitivní deficit je jedním z jádrových příznaků schizofrenního onemocnění u většiny pacientů od první epizody psychózy po celou následující dobu trvání nemoci. Empiricky podložené testové vyšetření kognitivních schopností je tak žádoucí součástí klinické praxe. V českém kontextu donedávna chyběla adaptace baterie MATRICS ve světě standardně používané pro vyšetření kognice u schizofrenie, stejně tak jako test Londýnské věže (ToL) - základní test pro vyšetření exekutivní schopnosti plánování. Hlavním cílem disertační práce bylo zpřístupnit tyto metody české odborné veřejnosti pro výzkumné i klinické použití. Tento cíl se uskutečnil u ToL i MATRICS baterie zjednodušeně ve dvou krocích: i) byla provedena adaptace české verze ve smyslu překladu testových instrukcí, způsobů administrace a skórování, podnětového materiálu; a ii) byla provedena komplexnější psychometrická studie ToL i MATRICS, v níž byla mimo jiné ověřena validita českých verzí těchto metod k diagnostice kognitivního deficitu u schizofrenie. Z výsledků vyplývá, že ToL je vhodným nástrojem k diagnostice deficitu plánování obecně, a u pacientů se schizofrenním onemocněním konkrétně, a že schopnost plánování měřena ToL je u pacientů se schizofrenií v průměru až o téměř o jednu standardní odchylku nižší oproti zdravým osobám. Součástí studie... In majority of schizophrenia patients, from the first episode of psychosis throughout the entire span of the illness, a cognitive deficit is one of the core symptoms. Assessing cognitive performance with empirically based methods is a vital part of clinical practice. Until recently, Czech adaptations of the MATRICS battery (standardly used for assessing cognition in schizophrenia worldwide) and the Tower of London, ToL (a basic test measure of executive planning ability) were absent. The primary aim of this dissertation thesis was to provide these methods for research and clinical use in Czech professional environment. This goal was achieved with both ToL and MATRICS in two basic steps. Firstly, Czech adaptations of both tests were carried out, namely translation of test instructions and establishment of standard administration and scoring. Secondly, complex psychometric analyses were conducted for both tests, including validation of their Czech versions for the purpose of assessing cognitive impairment in schizophrenia. The results suggest that ToL is a suitable tool for recognising executive planning deficit in general as well as in schizophrenia patients specifically.
