(CZ) Výzkum na poli obezity, diabetes mellitus a jejich komplikací se v posledních letech stále více orientuje na patofyziologické mechanismy jejich vzniku a možnosti jejich ovlivnění. Cílem předkládané práce bylo prozkoumat vliv dvou odlišných intervencí - tubulizace žaludku a fyzické aktivity - na antropometrické, biochemické, hormonální parametry a mRNA expresi vybraných prozánětlivých faktorů v podkožní tukové tkáni společně s mRNA expresí v periferních monocytech u pacientek, které podstoupily tubulizaci žaludku. Do studie s fyzickou aktivitou bylo zařazeno celkem 15 obézních žen s arteriální hypertenzí, které podstoupily 3měsíční cvičební program, který zahrnoval 30 min aerobního cvičení třikrát týdně. Do druhé studie s tubulizací žaludku bylo zařazeno celkem 13 obézních žen, které byly sledovány po dobu 2 let po výkonu. Získané výsledky naznačují, že v obou studiích měly obézní ženy před intervencí zvýšenou mRNA expresi prozánětlivých cytokinů, adipokinů, chemokinů a chemokinových receptorů ve srovnání s kontrolními skupinami. Obě intervence vedly ke zlepšení antropometrických parametrů a systémového subklinického zánětu. Studie s fyzickou aktivitou neměla žádný vliv na krevní tlak, lipidový profil a relativní genovou expresi komponent renin-angiotenzin-aldosteronového systému a jiných... (EN) Research in the field of obesity, diabetes mellitus and their complications in recent years is increasingly focused on pathophysiological mechanisms of their onset and potential prevention and treatment. The aim of the present work was to evaluate the effects of two different interventions - sleeve gastrectomy and physical activity - on anthropometric, biochemical, hormonal parameters and mRNA expression of proinflammatory factors in subcutaneous adipose tissue along with mRNA expression in peripheral blood monocytes in patients who underwent sleeve gastrectomy. A total of 15 obese women with hypertension were included into the physical activity study. These patients underwent a 3-month training program, which included 30 minutes of aerobic exercise three times a week. 13 obese women were included into sleeve gastrectomy study and were followed-up for 2 years after surgery. Our results indicate that in both studies obese groups had at baseline significantly increased mRNA expression of proinflammatory cytokines, adipokines, chemokines and chemokine receptors relative to control groups.
