[cze][eng] Bakalářská práce nese název Kojení u žen po porodu císařským řezem. Práce je rozdělena na část teoretickou a část výzkumnou. Teoretická část se zabývá císařským řezem, kdy blíže popisuje, jak se císařský řez provádí, z jakých indikací je k němu přistupováno, a jaká anestezie může být využita. Dále je zde popisováno, jakou péči vykonává porodní asistentka o ženu před a po císařském řezu, ale jsou zde také zmíněny komplikace, které se mohou v souvislosti s císařským řezem objevit. Dále je v teoretické části zmíněno období šestinedělí, kde se zaměřujeme na změny, které žena v tomto období prodělává nebo může prodělat. Významnou částí teoretické práce je kapitola laktace, ve které popisujeme anatomii prsů, k čemu laktace slouží, jaká je její fyziologie, jaké je složení mateřského mléka, ale také výhody kojení, jak pro novorozence, tak pro ženu. Dále se zde zabýváme rozvojem laktace, a s tím související technikou kojení, ale i komplikacemi kojení a alternativními metodami, jak podávat stravu novorozenci. Poslední kapitolou teoretické části je bonding a rooming in, kde vysvětlujeme tyto dva pojmy, uvádíme, jak a za jakých podmínek lze bonding s rooming in provést. Také je zde zmíněn a vysvětlen termín Baby Friendly Hospital. Pro bakalářskou práci byl stanoven cíl, kterým bylo zjistit, jaké mají ženy zkušenosti s laktací po porodu císařským řezem. Výzkumné otázky byly určeny: Jaký dopad má porod císařským řezem na kojení žen? Kde nejčastěji vyhledávají ženy pomoc v případě výskytu problému při kojení? Výzkum byl proveden pomocí kvalitativního šetření, kdy byly provedeny hloubkové individuální polostrukturované rozhovory. Ty byly vedeny se dvanácti ženami od 42. dne do 6. měsíce po porodu císařským řezem. Ženy byly před začátkem rozhovorů seznámeny s tématem a účelem rozhovorů. Respondentky byly ujištěny o zachování anonymity a o nakládání se získanými informacemi dle etických práv a norem. Na základě analýzy dat byly stanoveny dvě hlavní kategorie s podkategoriemi. První kategorie Kojení po s.c. se zabývala, jakým způsobem probíhal císařský řez, jaká byla použita anestezie, kdy došlo k prvnímu přiložení, jak často byly ženy s novorozencem v kontaktu, kdy nastoupila laktace, zda měly nějaké komplikace s kojení, ale také, kdo a jak důsledně, jim podal informace v souvislosti s technikou kojení. Druhá kategorie Vyhledání pomoci se zaměřovala na to, jestli ženy vyhledaly pomoc při komplikacích s kojením, zda poskytnuté informace byly dostačující, ale také, zda ženy stále kojí, či nikoli. Z výzkumného šetření vyplynulo, že po porodu císařským řezem kojilo devět respondentek, a nekojily tři. Pouze u dvou respondentek ze dvanácti se nedostavila laktace vůbec. Dále bylo zjištěno, že o technice kojení podávají nejčastěji informace porodní asistentky, kdy uvádějí smysluplné a dostačující informace. Z výzkumného šetření vyplynulo, že u devíti respondentek z dvanácti se objevila nějaká komplikace s kojením, nejčastěji jimi byla laktační krize a zánět prsu. S těmito komplikacemi ženy nejčastěji vyhledaly pomoc u obvodního gynekologa, který jim poskytl pro ně dostačující informace. Nejčastější odpovědí však bylo, že ženy nevyhledaly pomoc žádnou, takto ale odpovídaly respondentky, u kterých se komplikace neobjevila (R6, R8, R9) nebo respondentky, které odcházely již z porodnice bez laktace (R1, R2). The bachelor thesis is called Breastfeeding of woman after undergoing cesarean section. The thesis is divided into a theoretical and research part. The theoretical part deals with the theme of the cesarean section, it describes how the cesarean section is performed, what are the indications of it, and witch anesthesia can be used. Secondly it describes the care of a wife before and after the procedure which is carried out by a midwife. Another thing that is mentioned in this part are the complications of a cesarean section. Fourthly the thesis specifies the changes of a woman's organism during the period of puerperium. The next section that I would like to mention is lactation, where the anatomy of the woman's breast, physiology, composition of breast milk and the benefits for the women and the newborn are defined. In this section we are also concerned with the development of lactation, the related technique of breastfeeding, and the complications of breastfeeding and alternative methods of the feeding of a newborn. The last chapter of the theoretical part is the section of bonding and rooming-in, how and when it can be provided. The term "Baby Friendly Hospital" is also explained in the mentioned chapter. For this thesis was set the goal, what experiences have women with lactation after caserean section. The research questions were determined: What impact has Caesarean delivery on breastfeeding? Where does women find most often help in the case of occuring problems with breastfeeding problem? The research was conducted through a qualitative research strategy, using in-depth individual semi-structured interviews. They were lead with twelve women from the 42nd day to the 6th month after the caesarean delivery. Women were familiarize with the thesis and the purpose of the interviews before the interviews began. Respondents were assured of maintaining anonymity and handling information obtained in accordance with ethical rights and standards. Based on data analysis, two major categories were identified with subcategories. First categoty "Breastfeeding after caserean section" was conserned about progress of the section, what anesthesia was used, when the bonding was made, how often was the newborn in contact with woman, when the lactacion started, whether they had some complications with technique of breastfeeding, and who and how gave them information in connection of this technique. The second category "Help searching" was focused on whether women were looking for help with breastfeeding complications, whether the provided information was sufficient, but also whether or not women are still breastfeeding. The research shows that after the birth by caesarean section, nine respondents were breastfeeding and a three were not. Only two respondents out of twelve did not get lactation at all. Furthermore, it has been found that the most meaningful and sufficient information about breastfeeding techniques are most frequently provided by midwifes. The research also shows that nine respondents out of twelve had some complications with breastfeeding, most often with lactation and breast inflammation. With these complications, women most often sought help from a district gynecologist who provided them with sufficient information. However, the most frequent answer was that women did not find any help, but these answers were from respondents who did not have this sort of complication (R6, R8, R9) or respondents who left the maternity hospital without lactation (R1, R2).
