Moderní technologie, v čele se sekvenováním nové generace (NGS), mění řádově rychlost získávání genetických dat při odhalování genetických příčin vyvolávajících různá lidská onemocnění. Tím umožňují také změnu přístupu ke studiu a léčbě lidských nádorových onemocnění, která můžeme z řady důvodů chápat také jako onemocnění genetická. Práce si klade za cíl zmapovat vývoj sekvenačních metod DNA se zaměřením na NGS. Popisuje základní poznatky vedoucí ke vzniku současných sekvenačních postupů a jejich chemické, fyzikální a biologické principy. Ve stručnosti uvádí přehled způsobu zpracování získaných sekvenačních dat, jejich analýzy a využití postupů, které slouží pro predikce významu nalezených sekvenčních variant. Na vybraných příkladech demonstruje hlavní aplikace NGS v onkogenetické problematice, mezi které patří charakterizace somatických genetických změn u sporadických nádorů a identifikace hereditárních patogenních variant v nádorových predispozičních genech u nádorů dědičných. Tím ukazuje, jak jsou poznatky a použití metod NGS zaváděny do klinického výzkumu a lékařské praxe. This, for example, entirely changes the field of tumor biology research and treatment because malignant tumor could be also considered as a genetic disease. This work aims to describe development of the DNA sequencing techniques with a focus on NGS. We summarize the basic scientific achievements stimulating the NGS development and its chemical, physical, and biological principles. Further, we overview the main steps in sequencing data acquisition and analysis, and principles used by the software tools that are required for the prediction of functional importance in identified genetic variants. The importance of NGS for current oncogenetics is illustrated using selected examples, including characterization of somatic genetic changes in sporadic tumors and identification of germinal pathogenic variants in case of hereditary cancer syndromes, that demonstrate the rapid implementation of NGS technologies into the clinical practice.
