Byla vytvořena a plně validována nová metoda pro kvantitativní stanovení hydrochlorothiazidu a chlortalidonu, jako užívaných diuretik a antihypertensiv, v lidském krevní séru. Tato tandemová metoda spojující kapalinovou chromatografii a hmotnostní spektrometrii (LC/MS/MS) je jednoduchá, rychlá, přesná, správná a selektivní. Analyty a vnitřní standard sulfadimethoxin byly extrahovány extrakcí kapalina-kapalina (LLE) směsí ethylacetát-dichlormethan (80:20, v/v). Chromatografická separace probíhala na koloně C8 s reverzní fází. Jako mobilní fáze byla použita směs acetonitrilu s 0,2 % kyselinou mravenčí, eluce byla gradientová. Analyty byly kvantifikovány hmotnostním spektrometrem (MS) s lineární iontovou pastí a za použití ionizace elektrosprejem (ESI). Ionizace diuretik proběhla negativním napětím, vnitřního standardu v pozitivním modu. Metoda je lineární v rozsahu 0,5-200 ng/ml pro hydrochlorothiazid a 0,5-500 ng/ml pro chlortalidon. Byla ověřena přijatelná přesnost a správnost u QC vzorků, jejichž koncentrace byly v rozsahu kalibrační křivky. Optimalizovaná a validovaná technika, popsaná v této diplomové práci, byla zavedena do klinické praxe na úseku toxikologie v Ústavu klinické biochemie a diagnostiky ve Fakultní nemocnici v Hradci Králové. A new method was developed and fully validated for the quantification of hydrochlorothoazide and chlorthalidone, both diuretic and anti-hypertensive agents, in human serum. This liquid chromatography-tandem mass spectrometry method (LC/MS/MS) is rapid, simple, accurate and selective. The analytes and the internal standard were extracted by liquid-liquid extraction (LLE) with ethylacetate-dichlormethane (80:20, v/v). The chromatographic separation was performed on a reversed-phase column C8 with a mobile phase acetonitrile-formic acid (0,2 %), the elution was gradient. The analytes were quantitated by linear ion trap mass spectrometry with an electrospray ionization interface (ESI). The diuretics were analysed in negative ion monitoring mode, the internal standard in positive ion monitoring mode. The assay exhibited a linear dynamic range of 0,5-200 ng/mL for hydrochlorothiazide and 0,5-500 ng/mL for chlorthalidone in human serum. Acceptable precision and accuracy were obtained for QC samples with concentration over the standard curve ranges. This developed and validated method has been used in the toxicological laboratory in the Institution of Clinical Biochemistry and Diagnosis in Hradec Králové.
