Obstetric interventions and pelvic floor disorders - Abstract One of the principal objectives of obstetric interventions in the second stage of labor is prevention of pelvic floor trauma and associated pelvic floor disorders. The most commonly used and also most frequently discussed interventions are manual perineal protection and episiotomy. The majority of pelvic floor disorders are of subjective nature. Therefore, tools for objectification and severity quantification of these disorders are the key to reliable comparison and identification of the most effective interventions. The aim of the dissertation was to find the most effective modification of manual perineal protection, to compare two frequently used types of episiotomy regarding all pelvic floor disorders and finally to find a consensus on the most suitable instrument for anal incontinence severity assessment. Our studies concerning manual perineal protection experimentally described the direction and extent of perineal deformation during vaginal delivery. Urinary Incontinence in Women and the Treatment by Physiotherapy

This bachelor thesis is dedicated to the female urinary incontinence and its treatment by physiotherapy. The aim of the thesis is to provide a comprehensive overview of the methods and possibilities of physiotherapy in the treatment of urinary incontinence in women and to determine the effect of physiotherapy for urinary incontinence in selected patients. The theoretical part describes the functional anatomy of the pelvic floor muscles and their involvement in deep stabilizing muscles, as well as classification, etiology and prevalence of urinary incontinence, pathophysiologic mechanism of urinary incontinence and physiotherapy approaches that are used to treat the urinary incontinence. The practical part contains two case reports of patients diagnosed with urinary incontinence.
Physiotherapy in prevention and therapy of perineal injuries and episiotomies

This bachelor's thesis summarizes the information of pelvic floor function and perineal injuries that are caused by the delivery. In the theoretical part of this thesis are described anatomical structures in the pelvic area and their changes during the pregnancy and the puerperium. The main part of the thesis is focused on possibilities of preventing the perineal injuries. There are described problems that are caused by perineal lesions and dysfunction of the pelvic floor. Options of physiotherapeutic treatment of these problems are discussed as well. The practical part of the thesis is supplemented with a case study of a pregnant patient, who was preparing to the delivery during pregnancy with the goal of preventing perineal injury. The case study is completed with anamnestic data collection and analysis of kinesiology of the patient in pregnancy and after the delivery. A therapeutic plan is a part of the case study as well. The thesis is supplemented with the results of executed questionnaire survey of 49 patients after vaginal delivery.

9 Abstract Title: Influence of Power Plate machine vibrations on m. levator ani contraction. Objectives: In my thesis I would like to follow up on my bachelor`s thesis in which I was analyzing the importance of exercise on the Power Plate machine from a theoretical point of view. I set the objective of my thesis, in co-operation with professionals in this area, to find out whether Power Plate machines have any impact on the pelvic floor contractions, especially on the m. levator ani. Methods: Theoretical part of my thesis was conducted using the research (exploration of facts) method. For theoretical input I mainly used my bachelor`s thesis which was being followed up with. Majority of studies regarding WBVT (whole body vibration training) influence on human body is available from foreign sources. The practical part of my research was conducted via the quantitative quasiexperimental method. The works was further performed with use of the explanation (empirical and generally theoretical) methods and interpretation methods. Options of physio therapy for pregnant and their effect on the functional problems associated with pregnancy

This work summarizes issues of pregnancy and postpartum in the context of the pelvic floor. It Briefly describes important anatomical structures and their changes during pregnancy and after childbirth. It introduces readers to the usual course of pregnancy and childbirth and difficulties that may arise in these periods as a result of pelvic floor dysfunction of various causes. In this work there are integrated knowledges of the suitability and contraindications of physical activities during pregnancy and after childbirth, as well as suggested possibilities of physical activities. Normal practices in physiotherapy and physiotherapy methods and concepts that can be used during these periods are further described. Case report of pregnant woman in her last month of pregnancy and one month after childbirth, which was performed with anamnestic data collection and analysis of kinesiology is part of this work. The biomechanical effects of levator ani muscle laceration injuries after vaginal delivery

Title: The biomechanical effects of levator ani muscle laceration injuries after vaginal delivery Objectives: The aim of this thesis is to sum up current knowledge about the normal structure and function of levator ani muscle and findings about dysfunctions of a pelvic floor. By means of biomechanical instruments to analyse how the lower part of levator ani muscle closes (puborectalis muscle) during unilateral avulsion lesion. On the basis of computer simulation of biomechanical properties of the pelvic floor during avulsion lesion to describe the theoretical solutions for the therapy of the main types of dysfunctions of the pelvic floor. Methods: Biomechanical analysis was used with a help of computer model of muscular pelvic floor with a help of the ABAQUS model. This model with matched properties of muscular tissue was influenced by the standard pressure which influences the pelvic floor for a person of 80 kilos standing at rest. This model simulated unilateral avulsion lesion and biomechanical variables were observed during compensatory activation of uninjured parts of levator with the activity 100%, 50% and 10%.
